Identity Theft Rome Woman Accused of Getting GA ID Under a Different Name

A Rome woman accused of getting a Georgia ID under a different name and defrauding Blue Cross/Blue Shield was arrested Monday, according to Floyd County Jail reports.

According to the reports:

Elvira Bartolon Morales, 30, of 2010 Hull Ave., was charged with felony aggravated identity fraud and felony insurance fraud.

Morales fraudulently obtained a Georgia ID under the name Enna L. Soto and gained employment and medical insurance in 2007 through Sara Lee Bakery.

She then used the company insurance to pay for her visits to Harbin Clinic’s Women’s Center for pre-natal care in March.

The total amount for services came to approximately $1,097 and Blue Cross/Blue Shield paid approximately $435.25.

Morales remained in jail Monday night without bond and on a hold for U.S. Immigration and Customs Enforcement.

Wrongful Death Knox County, KY Man Shot by Caregiver

Police have identified the man shot to death in Knox County early Tuesday morning.

Police say Earl Carter, 75, suffered multiple gunshot wounds inside his home along J Goodin Branch Road in Barbourville. He was pronounced dead at the scene.

Police arrested Charlene Sue King, 49, and charged her with murder. Investigators say she served as the Carter’s caregiver.

Accident Reconstruction Lexington Bus Crash Sends 3 to the Hospital

A busy Lexington road was shut down for a few hours late this afternoon after a driver lost control of a car and sideswiped a Fayette County School Bus. The wreck sent three people to the hospital and fortunately the two adults and one student had non-life threatening injuries, it all happened on Todds Road, near the intersection of McFarland.

“The passenger car for reasons we don’t know yet, lost control and entered the on-coming lane and struck the school bus,” said Lt. Dean Marcum, Lexington Police Department.

Police said they don’t have reason to believe the driver of the red Toyota Solara was speeding or driving drunk at the time of the crash. On board the school bus – five children. The school district tells us the students were headed home from Edythe J. Hayes Middle School.

“The children were fine, they were talking to us. It’s a special needs bus and they were a little bit upset, but they did very well. They were especially glad to see their parents,” said Major William Saunooke, with the Lexington Fire Department.

An official with the school district said one of the students was taken to the hospital, by a family member. As for others injured in the accident, Police said the bus monitor and the driver of the car were both taken by ambulance to the hospital, with non life threatening injuries.

Tonight Superintendent Tom Shelton released a statement, “Our thoughts and prayers are with those who were involved in today’s accident. Each day our school buses transport thousands of students to and from school and their safety is always our top priority,” explained Shelton.

Authorities said the driver of the car will not be cited for the crash.

Fraud Investigation Wheelchair Supplier Indicted

A Laurel woman who fled the country on the heels of an D.C. Medicare fraud investigation has been indicted in a $500,000 scheme involving power wheelchairs and incontinence supplies, according to charging documents.

Patricia Mubanga Chisanga, 33, the chief operating officer for Emerald Medical Services, disappeared last summer, shortly after federal agents visited her office in Northwest Washington, court documents indicated.

Uche Ben Odunzeh, the owner of the business and the father of her child, pleaded guilty in October to one count of conspiracy to commit health care fraud.

According to court papers, two weeks after agents handed Chisanga a letter from the U.S. Attorney’s Office for D.C. stating that she was under investigation for federal health care fraud, law enforcement officials received a tip that she and Odunzeh were planning to flee the country.

Federal agents learned that Chisanga had obtained a one-way ticket to Ethiopia from Baltimore-Washington International Thurgood Marshall Airport for a flight that departed earlier that day. She was accompanied on her flight by a minor child with the last name Odunzeh.

Agents also discovered that Ben Odunzeh, 31, also of Laurel, had bought a ticket to Nigeria for later in the week. He was arrested at the airport.

According to prosecutors, from 2008 to 2011, Emerald Medical submitted 100 claims to D.C.’s Medicaid program for power wheelchairs for about $591,000. The D.C. Medicaid program reimbursed Emerald about $480,000.

Those claims were for the most expensive power wheelchair, worth about $6,100 each, but Emerald provided only more basic, less expensive wheelchairs.

Emerald Medical also submitted about $45,000 in claims for adult incontinence supplies such as diapers, disposable underpads and gloves that it never actually provided.

Odunzeh, a Nigerian national whose visa expired in 2004, admitted to embezzling more than $275,000 from the District of Columbia from 2008 to 2011. He faces 10 years in prison at his sentencing scheduled for Jan. 16

Private Detective Man Arrested for Placing Child in Cage

A Tulsa man was arrested after police found his 18-month-old daughter locked in a metal dog cage, his naked 4-year old daughter outside on a cold afternoon and him asleep in a drug or alcohol “induced stupor,” police said Monday.

A neighbor called police after finding the older child outside the family’s home and getting no response when he knocked on the door around 4 p.m. Sunday, as temperatures dropped to the low 40s. Officers’ knocks also went unanswered, “then one of the officers looked inside and saw a small child in a dog cage,” Tulsa police spokesman Officer Leland Ashley said.

After forcing their way inside, police discovered the toddler was covered in feces and that a 3-year-old child was asleep in another room. Their father was found asleep on a bed, “in an alcohol- or narcotics-induced stupor,” Ashley said.

The three children were placed into the custody of the Department of Human Services, and the father was arrested on a child neglect complaint. The Associated Press is not naming the father so to protect the identity of the children, who showed no sign of physical injury, Ashley said.

Their mother had been at work and arrived as officers were preparing to leave the scene. She was not arrested and her name was not released, Ashley said.

“A citizen was concerned, he heard a small child crying outside for about 20 minutes. He went outside to investigate and found the 4-year-old child,” he said. “You have to give credit to that neighbor who acted and did not ignore the situation.”

Messages left for the department and the Tulsa County District Attorney’s Office went unreturned Monday, a federal holiday.

The father was being held in the Tulsa County jail on $50,000 bond and his court date of Nov. 19, according to online jail records. No attorney was listed for him. He also had no listed telephone number.

A man by the same name and the same birth date was given a suspended sentence in August after pleading guilty to possession of a precursor to manufacture methamphetamine.

Missing Person Missing Knox County Man’s Body Found

Three men are accused of murdering a young Knox County soldier, who recently served overseas in Iraq.

The body of Jonathan “Ryan” Foley, 26, was found Sunday night in an abandoned strip mine near the Trosper community.

“I just miss him real bad,” said Greg Foley, the victim’s brother. “It’s just hard right now. I just keep picturing him in my mind. It’s hard to sleep.”

The 26-year-old was last seen Tuesday, when his family said he left home to get gas, but never returned. Shortly after Foley’s disappearance, his pickup turned up in a Barbourville factory parking lot, covered in mud.

“It’s been real hard on me because he’s my baby brother,” Foley said. “His birthday was the 9th. He’d been 27. He didn’t make it.”

Greg Foley went on to say the family is thankful for the three arrests made in the investigation. John Chris Mills, George Marcum, and James Wyatt are all charged with Foley’s murder.

“They murdered him. They need to pay for it. And I mean, they took a man’s life. They need to get their life took from them,” said Foley.

An autopsy was performed Monday morning on Jonathan “Ryan” Foley. Investigators have not released a preliminary cause of death or a motive for the crime.

All three murder suspects declined to talk to LEX 18 from the Knox County Jail.

Missing Person 15 Yr Old Girl of Bedford, TX

The girl you see at right is 15-year-old Amber Skye Mickelson of Bedford, who’s been missing since 8 p.m. Friday. According to Bedford police, that’s the last time Amber’s mother saw the girl, which is why police have issued a missing person alert. So far, an Amber Alert has not been issued.

Lt. David Smith, in Bedford PD’s Criminal Investigations Division, says that’s because it doesn’t appear at the moment that Mickelson was kidnapped. Says Smith, Amber’s mother initially reported her as a runaway Saturday morning. The 15-year-old has left home a few times before, says Smith, “but she hadn’t returned as she had in the past. There’s no evidence to say she was abducted.”

Per the Bedford PD’s alert: Mickelson is about five-foot-one, weighs 100 pounds, and wears braces and glasses. And she was last seen wearing black jeans, a green-and-black shirt and tennis shoes. Fraud Investigation KY Fallen Soldier Scam

Many people reflected on the sacrifices of our veterans, but a Mt. Sterling woman put out a word of warning.

Someone is apparently trying to cash in using Montgomery County hero Dustin Gross’ name. Jennifer Moyer says someone made a flyer asking people to donate $12.00 to the Dustin Gross Fund. The problem is, it didn’t come from their organization.

Moyer says someone recently notified her that they received a letter in the mail. But the organization says they only operate through the media and the Operation Dustin Gross Facebook page.

Their organization collects donations so that they can put together care packages for Kentucky units serving overseas. Things like toiletries, non-perishable foods, snacks and DVD’s and Cd’s.

Dustin died in May of this year while serving in Afghanistan.

“It’s sad that someone can take advantage of a good thing and turn it to evil. It breaks my heart that someone would do something like this,” says Jennifer Moyer with Operation Dustin Gross.

So far, Moyer says she’s only been contacted by a recipient of one of the letters, but that’s not to say more weren’t sent.

Arson Investigation 2nd Suspect Arrested in Clark County Fire

Police in Clark County have arrested a second man charged in an ongoing arson investigation.

Investigators say Brandon Guy and Aaron Spencer torched Kenneth Payton’s home back on Oct. 21.

Investigators say Payton was apparently a victim of assault prior to the fire. According to the Winchester Sun, detectives testified that Payton’s long-time girlfriend, Pamela Cantor, told them that she and the three men were drinking inside the home and there was a fight that night.

Police continue to investigate and have not filed any charges in connection with Payton’s death. Investigators say autopsy results are still pending.

Missing Person Missing KY Man Found Floating in Creek

FLOYD COUNTY, Ky. (WSAZ) — The body of a man, who was reported missing on Saturday, was found floating in an eastern Kentucky creek Sunday afternoon.

The Floyd County Coroner says 38-year-old Matthew Wayne Wright’s body was found near a railroad bridge in Frasure’s Creek in McDowell.

A person who was searching for Wright found the body.

Wright’s body was sent to Frankfort for an autopsy.
