Insurance Fraud Esther Obrzut Submitted Fake Chiropractic Treatments

Authorities say a Phoenix woman accused of defrauding a health and life insurance company of nearly $690,000 in fake chiropractic treatments has been sentenced to prison time and probation.

Arizona Attorney General Tom Horne announced Monday that 42-year-old Esther Obrzut got a 21/2-year prison term for forgery followed by five years’ probation for fraudulent schemes and artifices.

Obrzut also was ordered by a judge to pay more than $653,000 in restitution to Cigna Health and Life Insurance.

Prosecutors say Obrzut billed Cigna for $1.1 million for nearly 4,500 chiropractic treatments for herself, her spouse and her two minor children between May 2003 and February 2011.

Arizona Department of Insurance investigators determined that almost $685,000 was paid out by Cigna for treatments never received by Obrzut or her family.

Wrongful Death Sarah Hart UPDATE

There are new and disturbing details in the case of a man accused of murdering a pregnant Russell County mother.

Christopher Allman is charged with murder, fetal homicide, kidnapping, robbery, evidence tampering and being a persistent felony offender. Police say Allman strangled 31-year-old Sarah Hart while she was jogging along U.S. 127 near Jamestown in June. In court Wednesday, Allman was also indicted for allegedly raping Hart.

The mother of three was believed to be about 12 weeks pregnant at the time.

Allman will face the death penalty in the case, prosecutors said in July.

Missing Person Freddie Grant Arrested in Missing Person Case

A 52-year-old man has been charged with kidnapping a missing 15-year-old South Carolina teen after her blood was found on duct tape near his home.

Freddie Grant, was uncooperative when investigators asked him if he knew where Gabrielle Swainson, the teen who has been missing since Saturday, was, reports the San Francisco Chronical.

Gabrielle who lives at 221 Tamara Way in the North Crossing neighborhood of Columbia, S. C. has been missing since Aug. 18. She was last seen by her mother, Elvia Swainson, who left the house to go to work for a few hours. Gabrielle was asleep when she left, but when she returned Gabrielle was gone, reports Midlands Connect news.

Grant who dated Gabrielle’s mother was questioned by authorities early on in the investigation, and investigators became more suspicious of him when he refused to cooperate, said Richland County Sheriff Leon Lott.

Grant is a convicted felon and bullets were found in his home, leading to a federal weapons charge. Further search of his home lead to Gabrielle’s DNA found on some duct tape with blood on it near his home, reports the San Francisco Chronicle.

Also, cell phone records show that Grant was near Gabrielle’s home around the time she disappeared and both appeared to end up at Grant’s home, the sheriff said.

Investigators are also taking a closer look at the case of a missing woman that lived with Grant in Elgin. Adriana Laster, 28, was reported missing by her grandmother in March. Elgin Police Chief Harold Brown said that police were called to Grant’s home several times for domestic violence.

Currently, there is a reward for $6,000 for information leading to the whereabouts of Gabrielle.

Identity Theft Veronica Astorga Carmona Admits to Stealing 15 Identities

A local woman admitted Monday that she stole the identities of 15 patients while working at an Oro Valley oncology office.

Veronica Astorga Carmona could receive probation when sentenced next month or between two and 8.75 years in prison.

On June 25, a cancer patient gave his Visa credit card to Carmona following an appointment at Arizona Oncology, 1521 E. Tangerine Rd., according to court records. Later that day, he discovered his credit card had been used to pay Carmona’s rent of $983.25. He also discovered Carmona had used the card to buy a speaker system at Best Buy for $350.

Detectives searched Carmona’s house and found items that had recently been purchased at Lowe’s, JC Penney and Aaron’s Rent-to-Own, along with credit cards in other women’s names, court documents state.

She took the names and personal information of Arizona Oncology patients to obtain the credit cards, Carmona told detectives.

Carmona was indicted July 17 on seven counts of taking the identity of another, one count each of aggravated taking the identity of another and fraud.

On Monday, she pleaded guilty to one count of aggravated taking the identity of another. The count lists 15 individuals, along with Capitol One, Wells Fargo and Bank Of America, as victims.

Carmona told Pima County Superior Court Judge Howard Hantman she had gotten into a financial “bind.”

Knowing felons aren’t allowed to vote, Carmona asked Hantman to postpone her sentencing hearing until after Nov. 6.

Hantman set her sentencing for Oct. 1.

Private Detective Sumerset Couple Arrested

The Pulaski County Sheriff’s Department has arrested a Somerset couple on drug charges.

On Tuesday, the Sheriff’s Department Narcotics Division conducted an investigation into complaints of illegal drug activity at a duplex on Trails End Drive in the Slate Branch community of Pulaski County. That investigation led to the execution of a search warrant at the home.

During the search, detectives confiscated marijuana, heroin, several firearms and more than $2,000 in cash.

Chad Edward Puchalski, 25, and Brandy P Brown, 21, both of Somerset, were arrested and charged with trafficking in a controlled substance first degree, enhanced with the use of a firearm.

Both are being held in the Pulaski County Detention Center.

Wrongful Death 10 Month Old Baby Found Dead

The mother of a 10-month-old baby and her boyfriend have been arrested after the child was found dead inside a Laurel County home.

Police say they responded to a 911 call on Monday night that a 10 week-old-infant baby girl had been found dead at about 10 p.m. at residence on Somerset Road, located about two miles west London. They tried to resuscitate the child, but were unable to.

Deputies conducted an investigation, and determined that the baby had been left with the mother’s boyfriend’s brother early on Saturday. The mother and her boyfriend told the boyfriend’s brother that her other child had been critically hurt in a motorcycle accident and was in the hospital in Tennessee, and they needed him to watch the baby.

It was determined that the child had not been hurt, and that the mother and boyfriend had in fact been partying for 2 days. The boyfriend’s brother had no experience in caring for a newborn infant baby.

Deputies located the mother and her boyfriend later in the night driving in the vicinity. The boyfriend ,Johnathon L Haynes, 32, was arrested and charged with DUI, and several other traffic offenses. Also taken into custody was the mother of the baby, Rachel E. Kilburn, 29, of Cutshin, Ky

Following investigation, she was charged with: abandonment of a minor, a class D felony and is being held at the Laurel County Detention Center. Haynes is also being held.

An autopsy is being conducted in Frankfort to determine cause of death. Police are continuing to investigate.

Missing Person Jesse McMillen, 21, A Penn State Student

State College police are searching for Jesse McMillen, 21, a Penn State student who was last seen at the Red Carpet Inn in Ebensburg when he told the desk clerk he was hitchhiking to see a friend in Pittsburgh, according to reports.

Although police said the friend in Pittsburgh has been identified, McMillen has not yet been seen. Supposedly he was looking for a place to rent for the upcoming school year and was thought to have been looking at a camping trailer somewhere near Bellefonte.

McMillen, whose parents live in Lake Ariel, is described as five feet seven inches tall and 180 pounds, with brown hair and brown eyes and glasses.

Police said he went to look at a camping trailer he found on Craigslist, and the trailer was allegedly owned by a woman names Nan.

Accident Reconstruction Teen Airlifted to Hospital

A teenager was airlifted to a Tennessee hospital after a crash in Harlan County involving a garbage truck.

The incident happened at about 9:40 a.m. Tuesday on U.S. 119. Police say a 2000 Chevrolet pickup driven by Ronnie Clark, 17, of Cumberland was attempting to turn left from Harlan County High School parking lot onto U.S. 119. At that time, the pickup crashed into a 1996 Chevrolet garbage truck traveling south on U.S. 119.

The driver of the garbage truck, Paul Smith, 53, of Lynch, was not injured. He was wearing a seat belt.

Clark was trapped inside the pickup and was removed by emergency personnel. He was not wearing a seat belt. He was airlifted from the scene by AirEvac to Holston Valley Medical Center in Kingsport, Tennessee.

Alcohol or drugs are not suspected. Blood was obtained from both drivers.

The crash remains under investigation.

Missing Person Jeffrey Wachholz of Florida

JACKSONVILLE, Fla. — The Florida Department of Law Enforcement has issued a Silver Alert for Jeffrey Wachholz.

The alert was issued after police responded to the 3800 block of Bettes Circle to investigate reports of a missing person.

Wachholz, 50, was last seen Saturday morning at 10:16, according to a news release from Jacksonville Sheriff’s Office Public Information Officer Shannon Hartley.

Hartley said Wachholz was seen leaving his home. Wachholz was driving his 2000 gray convertible BMW Z3 with Florida tag “GR8FACE.”

Wachholz was supposed to travel to New York for a mental evaluation. Hartley said he left to avoid making the trip.

The victim, according to police, suffers from depression and early onset dementia.

Insurance Fraud Maria Schebece of Hillsborough

SOMERVILLE — Maria Demeter, also known as Maria Schebece, 42, of Hillsborough was charged Monday with insurance fraud, third degree, Somerset County Prosecutor Geoffrey D. Soriano said.

Along with Somerset County Prosecutor’s Office Chief of Detectives Stuart A. Buckman, Soriano said that in April, the Somerset County Prosecutor’s Office Insurance Fraud Unit received a referral from New Jersey Manufacturer’s Insurance Company, which indicated that one of its insureds by the name of Maria Demeter recently had disclosed that she made material misrepresentation in obtaining her motor vehicle insurance policies since 1999. Specifically, Demeter had obtained two motor vehicle policies as a single woman living in a household with no other registered drivers. However, this was false as she was married and living with her husband, a registered New Jersey driver.

The investigation revealed that due to Demeter’s misrepresentations, she obtained an insurance policy with NJM that she would have in fact not been eligible for due to her spouses’ driving record, Soriano said. Demeter also obtained a monetary benefit by paying substantially lower insurance rates than she would have paid had she provided true information to NJM.

Demeter was served with a summons Monday by detectives from the Somerset County Prosecutor’s Office Insurance Fraud Unit and released on her own recognizance, Soriano said.
