Identity Theft Blind Woman Falls Victim to ID Theft

A 21-year-old woman is charged with numerous identity theft and fraud counts after allegedly draining a blind woman’s bank account while helping her run errands last year.

Investigators believe Chamika Emmalia Fujioka was paid to help the blind woman run day-to-day errands such as trips to the grocery store and the bank.

On several of these bank runs, Fujioka was tasked with withdrawing money from the woman’s account.

“She had the woman’s PIN and would basically do double transactions,” Ashland police Sgt. Jim Alderman said.

Fujioka would withdraw the money the woman asked for and money for herself, police said.

“She did this in multiple occasions and the woman didn’t find out about it until later,” Alderman said. “She would get the woman her $40 and then withdraw $400 for herself.”

The crimes reportedly occurred last fall. The victim has since moved to the Portland area. Once she discovered her bank balance was lower than it should be, she called the police.

Fujioka’s case is scheduled to appear before a Jackson County grand jury soon. She is charged with 19 counts of identity theft, one count of aggravated identity theft, eight counts of first-degree theft, eight counts of felony credit card fraud, 19 counts of misdemeanor credit card fraud and 17 counts of second-degree theft.

Investigators allege the victim lost somewhere between $6,000 and $8,000 during the period in which the crimes occurred.

At this point, police have found no evidence that there were any more victims involved in the case. Fujioka is not cooperating with the investigation, Alderman said.

Police advice that anyone who seeks to hire someone to help them with various chores should perform a background check on the applicant and install a checks-and-balances system that might catch fraudulent activity before the loss becomes too large.

It is also a good idea to have your bank inform you of any suspicious transactions, such as numerous withdraws made on one day.

“It all comes down to being sure you know who you’re handing your credit card to,” Alderman said.

Pedophile Tracking Woman Sentenced to 80 Years

The second person found guilty in connection with the sexual abuse of a young girl received an equally harsh sentence in Harford County Circuit Court Wednesday to match the one given last month to the other defendant in the case.

Angeline Kaye Feazelle was sentenced by Circuit Judge Stephen Waldron to a total of 80 years in prison, to be served consecutively. Feazelle, 31, of North East, was convicted by a jury on Jan. 28 of sex abuse of a minor as a continuing course of conduct and two counts of sex abuse of a minor. Waldron sentenced her to 30 years on the continuing conduct charge and to 25 years on each of the sex abuse of a minor charges.

Feazelle has been jailed without bail at the Harford County Detention Center for most of the time since her initial arrest on these charges in February 2012 and, as of sentencing, had served 321 days, according to court records.

Jeremy Shane Cochran, 32, of the 4100 block of Federal Hill Road, was found guilty in December by a Harford County Circuit Court jury of sexual abuse of a minor as a continuing course of conduct, sexual abuse of a minor and conspiracy to commit sexual abuse of a minor, according to the case file.

Feazelle and Cochran lived together with the victim at Cochran’s house in Jarrettsville, where the abuse occurred, according to their court files. The prosecutor in the case said the abuse started when the victim was 5 years old.

Last month, Waldron sentenced Cochran to 30 years on the continuing course of conduct charge and 25 years for each of the other two charges, a total of 80 years to be served consecutively. Cochran also had been held in jail without bail since his arrest in early 2012.

The sentences are believed to be among the harshest ever meted out in Harford County for crimes that did not involve homicide. Feazelle and Cochran got what they deserved, according to Deputy State’s Attorney Diane Adkins Tobin, who prosecuted both of them.

Cochran was the abuser, but Feazelle was a willing participant, Tobin said. The Aegis has chosen not to disclose the relationship between the victim and her abusers in order to protect her identity.

“This is probably one of the most horrific cases of child sex abuse I have ever prosecuted,” said Tobin, who has been with the state’s attorney’s office since 2002.

“This abuse was ongoing and the victim was so young and she was trapped,” Tobin continued. “It was so horrible, and she was impacted so much. Her childhood was taken away from her, and she will have to deal with it afterward for a long time. It was just a terrible breach of trust.”

Tobin called the sentence for both abusers “appropriate and reasonable.”

“This girl’s life was taken away from her; there’s no reason either of them should be free,” she said.

She also said Cochran had committed “really horrible sex acts” on the victim, while Feazelle had failed to stop it.

“It’s hard for to choose which one of them was the worst,” she added.

Cochran and Feazelle were originally arrested and charged in 2008; however, those charges were dropped after Tobin and others prosecuting the pair were advised the victim was “not in a position emotionally to testify,” Tobin explained, adding: “I just knew both cases would go to trial,” and thus the victim’s testimony would be necessary.

At that time, the victim was removed from the Jarrettsville home and went to live with a family member outside of Harford County. The charges were refiled last year after the victim had time “to heal a little,” Tobin said. According to the case files, the victim had received counseling during the period.

The victim, who is almost 10, according to Tobin, had not seen either Feazelle or Cochran for almost five years when she testified against both at both their trials.

“She was very brave; she wanted to face in court what they had done,” Tobin said, noting there was DNA evidence in addition to the victim’s testimony. “She is a very brave child.”

A south Charlotte family is asking for help to find a man who has been missing since Thursday.

65-year-old John Greene hasn’t been seen since Thursday night and his family members say he doesn’t have his heart medication.

Identity Theft 4 Tips to Guard Your Social Security Number

Your Social Security number is the key to your financial castle. Armed with those precious nine digits, identity thieves can rip off your good name and credit to set up new accounts and loans or rob your existing accounts.

High-tech schemes to steal your “Social” use deceptive phishing emails, spyware or keystroke software that copy your passwords for online banking or other accounts. Low-tech methods include stealing wallets and dumpster-diving for unshredded bank statements.

No matter how crooks get your number, the turmoil they will spin into your life could take you months and thousands of dollars to resolve. In 2010, 8.6 million U.S. households were touched by identity theft and lost $13.3 billion as a result, according to a Bureau of Justice Statistics survey.

“Of any piece of personal information, the Social Security number is the most critical to safeguard,” says Lisa Schifferle, an attorney in the Federal Trade Commission’s Division of Privacy and Identity Protection. “It’s the golden ticket to identity theft,” she says, adding that government watchdogs field more complaints about identity theft than anything else.

Here are four things you must know to protect your Social Security number.

Know Where You Need to Provide Your Social

Unless you’ve lived entirely off the grid and have never been employed, paid taxes, received government assistance, or had a mortgage or other loans, you’ve needed a Social Security number at some point.

According to Mark Hinkle, spokesman for the Social Security Administration, these are the entities that are most likely to require you to provide your number.

Employers.
Internal Revenue Service for tax returns and federal loans.
Banks and lenders.
U.S. Treasury for savings bonds.
Other government-funded programs such as welfare and workers’ compensation.

Keep in mind, too, that government agencies obligate certain businesses to acquire your Social Security number. For example, under IRS rules, banks must obtain a Form W-9, or Request for Taxpayer Identification Number and Certification, from customers opening new accounts, says Mark Rodgers, a spokesman for Citibank. The taxpayer ID that’s requested by a W-9 is the customer’s Social Security number.

At Wells Fargo, spokesman James Hines says new accounts requiring a taxpayer ID or Social Security number include savings accounts, loans and even safe-deposit box rentals. Wells Fargo does not put the numbers on statements or online accounts, he notes.

Know Who Doesn’t Need Your Number

You’ll find a blank space to fill in your Social Security number on many forms and applications.

That’s because it’s a number unique to you and therefore an especially easy way to identify you.

Schifferle and Susan Grant, director of consumer protection at the nonprofit Consumer Federation of America, say the following businesses commonly ask for Social Security numbers — especially if they run credit reports — but don’t have to have them.

Schools.
Sports leagues/clubs.
Utilities/cellphone companies.

Landlords/property managers.
Hospitals/medical offices.
Insurers.

Why do these businesses want your Social? Most landlords, for example, consider the numbers necessary for doing credit checks, which provide a treasure trove of information about prospective tenants, says Rebekah Near, CEO of Orca Information, a company in Anacortes, Wash., that handles employment and tenant screening nationwide. Property owners can learn whether prospective tenants are financially responsible and if they’ve ever been evicted or arrested.

“People don’t see why it’s absolutely necessary in every case,” says Near, also a regional official with the National Association of Residential Property Managers. “But people lie.”

Know You Can Just say ‘No’

To protect your Social Security number, you can always refuse to give it when businesses ask for it. But they can deny you service, says Schifferle.

Still, don’t just automatically fill in that blank on forms, says Grant. Be skeptical about why the business needs your number. She suggests offering your driver’s license number instead.

At credit bureau TransUnion, spokesman Clifton O’Neal says credit reports can be pulled without Social Security numbers. But the additional information can allow a business to receive a more complete report about you.

TransUnion screens tenants and doesn’t always need Social Security numbers to provide landlords with credit reports, O’Neal says.

Insurance companies might charge you higher premiums if you don’t supply your number, warns J. Robert Hunter, insurance director for the Consumer Federation of America.

But spokeswoman Michal Brower says major insurer State Farm doesn’t even ask.

Some state laws bar colleges and universities from demanding students’ Social Security numbers, except for financial aid, says LeRoy Rooker, a senior fellow with the American Association of Collegiate Registrars and Admissions Officers, or AACRAO.

Most schools store students’ numbers in a database but don’t use them on transcripts, an AACRAO survey found.

Ask a Ton of Questions

If you just can’t avoid giving out the number, “we advise people to ask a lot of questions,” says Schifferle. Start with these.

“Why do you need my Social Security number?”
“Will my number be shared with anybody?”Your number and other nonpublic personal information may be shared by your bank with affiliates and other businesses. But federal law requires companies offering financial products such as loans, insurance or investment advice to tell how they share your information.
“May I see your privacy policy?”A legitimate business requesting your Social Security number should have a privacy policy explaining why personal information is collected, how it’s used, and who will have access to it. The California attorney general’s office recommends that if you’re not satisfied with the terms of the policy or if there is no written policy, just walk away.
“How will my Social Security number be stored?”You want to know, for example, that your rental application won’t sit in a pile on the apartment manager’s desk that someone can sift through after hours. Feel confident that a business will take measures to protect your Social Security number.

Pedophile Tracking England Man Traveled to KY to Engage in Sex

A resident of London, England traveled to Kentucky intending to engage in sex acts with a minor who lived in eastern Kentucky according to a plea agreement filed in federal court.

Solomon Blue Waters, 42, of London, England pleaded guilty Wednesday before U.S. Magistrate Judge Edward B. Atkins to using the internet to engage in sexual activity with a minor. The U.S. Attorney’s Office and the defendant agreed to a proposed sentencing range of 135 to 168 months pending the judge’s approval. Waters is scheduled to be sentenced on September 5 in Pikeville.

According to the plea agreement, Waters admitted he developed an online relationship with the minor in 2011 and persuaded her to send him sexually explicit images of herself. Waters also encouraged the victim to communicate with him in sexual conversations. Waters planned to visit the minor in eastern Kentucky and he specifically described the sexual acts he would perform on the minor when meeting her.

On August 27, 2012, Waters flew from London, England to Lexington’s Blue Grass Airport and was immediately arrested by agents with Homeland Security Investigations and the Kentucky State Police.

Wrongful Death Man Faces Charges in Death of 5 Yr Old

A man now faces charges for the death of a five year old Lexington boy killed in a car crash Sunday night. Police say Alfonso Diaz- Diaz was driving drunk in Woodford County when he slammed into a car carrying the Cohorn family killing 5 year old Blake. Family and friends gathered to remember the life of the little boy as they rallied around Wendy Cohorn, his grieving mother, bruised and battered from the wreck.

“That’s the last thing I remember, is him telling me he loved me and that I was the best mommy in the world. And then I woke up in the hospital,” says Wendy, who was released from the hospital Wednesday.

Blake and his family were headed home from Indiana Sunday night when police say Diaz-diaz was driving drunk the wrong way on US 60 near Midway Road hitting them head-on. Diaz-Diaz now faces charges of manslaughter, assault, wanton endangerment and no operators license.

“He deserves to know every day for the rest of his life that he took the best thing in this world from me. He took my 5 year-old baby. He took my world,” says Wendy.

Family and friends released balloons, lit candles and left messages honoring Blake’s memory. His teacher, Katherine Zeh, says she was searching for closure.

“We all loved him like he was our family because he was our family and i loved him like he was my child and it’s just been a tough week. We lost one of our own,” says Zeh.

She’s had to explain Blake’s death to her young students, children like Emily Howard, Blake’s best friend.

“Maybe there was a reason that he died. Maybe Katlyn and Landon his little brother and sister needed a guardian,” says Howard.

“We will never forget Blake and he will always be a part of my heart and he took a little piece of my heart with him,” says Zeh.

Family member’s says that Blake’s father, Rocky Cohorn, is back in ICU. A fund has been set up in Blake Cohorn’s name at Fifth-Third Bank. Money will go to help pay for the little boy’s funeral.

Missing Person Search Continues for Dustin Self in Oregon

On March 16, the Harney County Sheriff’s Office received a telephone call from Victor Self of Oklahoma City, Okla. Self stated that his 19-year-old son, Dustin Rollin Self, had called him and stated that his GPS had guided him to the Fields-Denio Road in southern Harney County and that he was lost. Self asked for assistance finding his son.

Dustin had left Oklahoma and was enroute to the Klamath Falls -Ashland area to join a church, The Church of the Holy Light of the Queen, after a break-up with his girlfriend.

An investigation found that Dustin had made contact with the Fields Store, and that a customer had given him directions to Lakeview. A search was made by the Harney County Sheriff’s Office, with the assistance of Bureau of Land Management employees, with negative results.

Dustin’s family then had a missing person report filed and entered into the National Crime Information Computer system.

On April 15, a report was made to the Harney County Sheriff’s Office that a vehicle registered to Dustin Self was found on the Stone House Road at the north end of Steens Mountain by local area ranchers. The vehicle was located approximately two-and-one-half miles west of the Fields Denio Road, and had slid off the roadway. This location is 35 miles north of the area first searched.

Harney County Sheriff’s Office and Search and Rescue responded to the scene and conducted a search for Dustin with negative results. The search was hampered by severe storm conditions, high winds, and deep snow drifts in the area. An air search is planned when the high winds in the area subside. The area where the vehicle was found is described as very rough wilderness.

On Thursday, April 18, Sheriff Dave Glerup advised a local rancher searched the area in a helicopter with negative results as deputies continued with a ground search. As soon as weather permits, the Harney County Sheriff’s Office will launch a couple planes to help with the search for Dustin Self.

Identity Theft Montgomery, AL Man Sentenced to 51 Months

A Montgomery man was sentenced to 51 months in federal prison today after pleading guilty to aggravated identity theft and access device fraud early this year.

Kenneth Jerome Blackmon Jr. was involved in a scheme to use stolen identities to file false federal income tax returns with the Internal Revenue Service, the U.S. Attorney’s Office for the Middle District of Alabama released.

Blackmon admitted to acquiring names and Social Security numbers, to using that identity information on false tax returns, and to directing fraudulent tax refunds onto debit cards.

Blackmon also admitted to possessing at least 15 Social Security numbers for the purpose of obtaining fraudulent tax refunds from the IRS

In addition to prison time, Blackmon was ordered to pay $197,839 in restitution to the IRS and to serve three years of supervised release following his release from federal custody.

Assistant Attorney General for the Justice Department’s Tax Division Kathryn Keneally and United States Attorney for the Middle District of Alabama George L. Beck, Jr., commended the efforts of IRS – Criminal Investigation special agents in investigating the case, Tax Division Trial Attorneys Justin Gelfand and Jason Poole in prosecuting the case, and the Alabama Department of Pardons and Paroles, the Gwinnett County Sheriff’s Department in Georgia and the United States Marshal Service in assisting federal authorities with the investigation.

Missing Person Empty Car Found in Post Falls, ID

On April 24th, just after midnight, an officer with the Post Falls Police Department was completing a routine check of Q’emiln Park in Post Falls when he discovered a vehicle parked at the trail head. The vehicle was unoccupied and registered to a 70 year old female, Joan Gundlach.

The officer investigated further and made contact with the husband of the registered owner. It was discovered that Joan had been depressed and possibly suicidal when she left the residence on April 23rd at approximately 10:00 A.M. Joan had not been heard from since.

Additional units from the Post Falls Police Department along with assistance from the Kootenai County Sheriff’s Office, responded and began to search the park and the Spokane River. No evidence was found and Joan was not located. Post Falls Police Detectives responded and are continuing the investigation into the missing person case.

Joan was last seen around 10:00 A.M. however no clothing description is available at this time. Joan is a white female, approx 5’8″ and 155 pounds with brown hair and brown eyes. Wrongful Death 5 Dead in Shooting in Manchester, IL

Authorities have identified a man suspected of killing five people and injuring another in a mass shooting Wednesday morning in the Village of Manchester.

The suspected shooter, 43-year-old Rick O’Dell Smith, died later that morning following a shootout with authorities.

The murders happened inside a public housing complex on East Street around 4:30 a.m., according to ISP Trooper Mike Kindhart.

The five deceased victims were located in the residence: two pairs of victims were in separate bedrooms, and the fifth victim was found in the hallway. The age of the victims ranges from infant to adult. They have not been identified, but authorities said they were all related.

The victims were all killed with a shotgun at close range, according to ISP Lt. Col. Todd Kilby.

Investigators said witnesses told them Smith took the sixth shooting victim, a six-year-old girl, and handed her over to a neighbor. The six-year-old victim was rushed to a local hospital and remains in critical condition.

Police located Rick O’Dell Smith shortly before 7:15 a.m. in a vehicle. Smith exchanged gunfire with Illinois State Police and Winchester police at approximately 7:27 a.m. before being taken into custody. He was taken to a nearby hospital and later pronounced dead at 10:10 a.m.

Investigators have not disclosed a possible motive.
