Missing Person Anselmo Bahena of South Lawndale

A missing person alert has been issued for a 37-year-old South Lanwdale man who police say may be “confused.”

Anselmo Bahena is missing from the 2700 block of South Sacramento Avenue, according to the alert from Area Central detectives. He was last seen about 11 a.m. on Friday, according to the alert. Police say he may be ‘confused.’

Bahena is a Hispanic man with an olive complexion. He is 5-feet, 4-inches tall, and weighs 120 pounds, with brown eyes and black hair. When last seen, he was wearing dark brown and red pajama bottoms and a T-shirt.

Identity Theft Trio Behind Bars in Visalia, CA

A traffic stop initiated by Visalia narcotics detectives led to the arrest of three suspects, who police say stole thousands of dollars from major retailers by passing bad checks up and down the state.

Visalia narcotics officers stopped the vehicle in the area of Highway 198 and Highway 99 and discovered several forged checks and identity theft-related items in the car.

Officers also found numerous items purchased at local stores using the bad checks, according to police. The suspects, who police said were sophisticated in their techniques, are most likely a part of larger criminal fraud and forgery ring.

The suspects used forged business checks to buy things at Home Depot, Orchard Supply Hardware, Sears and Office Depot and then return the items for a cash refund at a location in a different city, according to police.

The estimated loss to the four businesses in Visalia was $4,000, but police said the suspects have been linked to crimes from Southern California to Fresno.

Officers arrested Luis Martinez, 29, and Frank Palomares, 29, both of southern California on suspicion of passing forged checks, possession of forged identifications, burglary and identity theft-related crimes.

“Being a criminal is their profession. It’s a way of life,” said Visalia Sgt. Amy Watkins. “They go up and down California doing this, and making money. When they get caught, they’re released and start all over.”

Narcotics officers worked with detectives from Fresno Police Department as well as detectives from Visalia’s property crimes unit to track down the suspects, who’d been staying in Fresno.

In hopes of finding additional evidence for prosecution, detectives served a search warrant later in the day Thursday at a Holiday Inn in downtown Fresno.

In the room, police found Marie Galaviz, 33, of southern California, as well as a computer, printer and external hard drive. All of which were seized by police.

Galaviz was arrested and booked on fraud, forgery and identity theft crimes.

She also had an outstanding warrant, according to police.

Police added that Palomares initially gave police a fake identification, but fingerprints taken during the jail booking process led to his real identity and showed he was a parolee-at-large.

Police had been investigating the suspects prior to the traffic stop that led to their arrest.

Fraud Investigation Man Arrested in Stratham, MA Faces 13 Charges

STRATHAM — A Massachusetts man arrested at an Osco Pharmacy in Stratham is facing 13 charges related to forged prescriptions.

N.H. State Police said James Whitmyer, 45, of 15 Donovan Drive, West Newbury, was arrested Wednesday, Sept. 26 at the completion of a monthlong investigation. Police allege he was trying to pass a fraudulent prescription at the time of his arrest in Stratham.

Whitmyer was charged with uttering forged prescriptions at multiple Rockingham County area locations.
The arrest was made by the N.H. State Police Narcotics & Investigations Unit, which credited assistance provided by Stratham police and Massachusetts State Police.

Whitmyer faces arraignment dates in N.H. circuit courts in Salem (Nov. 14), Plaistow (Dec. 10), Candia (Dec. 12), Seabrook (Dec. 13) and Brentwood (Dec. 14).

Missing Person Body Found in Detroit River

Wyandotte police said they responded to a call from the Coast Guard at about 1 p.m. Friday and recovered the body of a black male near Hennepin Point in the Detroit River.

Wyandotte police are saying that they believe the body found in the Detroit River Friday was a missing Dearborn man.

Police recovered the body of a black male from the Detroit River on Friday afternoon, the Wyandotte Police Department said in a statement.

Wyandotte and Grosse Ile police responded to a call at about 1:30 p.m. Friday from the United States Coast Guard after a body was spotted near Hennepin Point in the Detroit River.

The body was recovered and taken to the Wayne County Medical Office for identification.

The results have not yet been confirmed, but police told Patch Saturday morning that they believe it is the body of Dearborn resident Ramas Paull McCrory, and have notified the man’s family.

McCrory was named as a missing person on Monday after clothing and other items belonging to the him were found abandoned near the Detroit River at the north end of Bishop Park on Saturday. Police also found an abandoned pick-up truck, which McCrory’s girlfriend identified as his.

Identity Theft Ring Busted Near San Francisco Airport

South San Francisco – This high-tech digital age we live in provides us with so many conveniences, especially when traveling, communicating with friends and family or shopping. Yet Identity theft and scams of all kinds is a very real danger.
In fact as reported by South San Francisco Patch news, this past August an identity theft ring was caught on Gateway Boulevard at a hotel not far from the Bayshore Freeway and West Harris Ave. This is an area close to San Francisco Airport, less than a 25 minute drive south of the City of San Francisco.

One male and two females, ranging in ages from mid-20’s to late-30’s, had been using the hotel room as their center of operation. They were caught using one of the stolen credit cards they had along with many birth certificates, social security numbers and a portable credit-card reader machine. All three were arrested and booked into the San Mateo County Jail in Redwood City, thanks in part to one of the many victims alerting authorities that a credit card was stolen.
Sergeant Bruce McPhillips of the South San Francisco Police Department told this reporter, on assignment for the Peninsula Progress, the day after Labor Day, that he had no new developments in the case. But he did say briefly as spokesman for the Community Relations Division of the SSFPD, “people must be extra careful these days.”
To reiterate how “close to home” and wide-spread this type of crime is, The Huffington Post reported back in December of 2011, that 23 Northern California Lucky supermarkets were vulnerable to identity theft through their “self-checkout” machines.
Lucky supermarket officials first noticed the plot when routine maintenance was conducted at 19 locations. More than a dozen Lucky locations on that routine maintenance check were right here on the Peninsula. Locations as far as Santa Clara and including Lucky stores on Mission Street in Daly City and Edgewater Blvd in Foster City were affected. Thieves used credit card “skimmers” to obtain information and pin numbers from unsuspecting customers as they used the self-check out machines.
People often don’t think when they are in a hurry and convenience often wins out over some basic safety precautions. Sergeant McPhillips noted that no one is completely safe of identity theft as new technology advances, so does the way thieves use technology to steal and scam people.
Some simple rules to follow to help safeguard oneself from identity theft are, keep all accounts, personal and important information away from situations were access could be obtained. For example, avoid giving out your Social Security Number, ID numbers, maiden names, pin numbers or anything that might give a thief opportunity to steal your identity. Avoid leaving bills, receipts, etc., behind or in places where thieves can take that info and use it to access accounts or records.
With regards to machines, such as the “self-check out,” avoid any machine, including ATM’s that look messy or as if they have not been maintained in a while. (Especially so for free standing ATM machines, those are usually independently operated and are not maintained by a bank). Lucky supermarket officials say that all their self-check out machines are safe and working properly.
Sergeant McPhillips and others say that people should be vigilant and keep watch for any suspicious activity on their credit card or bank accounts. Never approach a thief; contact police immediately if you suspect you are the victim of identity theft.

Missing Person Baby LIsa Irwin Missing 1 Year

Kansas City Police said in a press release Friday that they have not yet had the opportunity to sit down one-on-one to speak with missing Baby Lisa Irwin’s mother, Deborah Bradley.

For months police have requested the one-on-one meeting with Deborah. The press release states:

“While communication with the [Irwin] family has been ongoing, police have not had the opportunity to sit down one-on-one to speak with Lisa’s mother, Deborah Bradely.

“As the only adult in the home at the time of the baby’s disappearance, police continue to have questions to which only she can provide answers.”

The press release further states:

“Police have exhausted leads provided by Lisa Irwin’s family and their attorneys, and the leads were of no benefit to the investigation.”

According to the press release, detectives continue to actively investigate the case as the one-year anniversary of Baby Lisa’s disappearance nears. KCPD and their FBI partners have investigated 1,667 tips since 10-month-old Baby Lisa went missing from her Kansas City, Missouri home in the 3600 block of North Lister in the early morning hours of October 4, 20122. They are checking into about a dozen tips at present.

Five hundred of the total tips received have been infant sightings across the U.S. and internationally. KCPD thanked FBI agents and other law enforcement agencies, domestically and overseas, who investigated each of the sightings. Unfortunately, none of them were determined to be Baby Lisa Irwin.

A KCPD detective and FBI agent actively work the case almost daily and are looking for quality tips. They have gone over nearly 100 previously closed leads and re-interviewed involved parties to ensure they had not missed something.

They and Crime Lab analysts also have re-evaluated several pieces of forensic evidence and dozens more detectives stand at the ready to investigate a break in the case.

Detectives remain hopeful that quality tips will lead them to Lisa. The Press Release addressed a long-standing issue in this and other missing persons’ cases – tips that stem from social media and online rumors that detract from detectives’ ability to work legitimate case leads.

They said that several tips in Baby Lisa’s case that stemmed from social media and online rumors were reported, and that those tips have caused a distraction that took detectives away from their work.

Arson Investigation Ricardo Dale Travis Charged

Potter County prosecutors have charged a man with arson after authorities said he started two separate fires at an Amarillo residence on Sept. 11, court records show.

Authorities said Ricardo Dale Travis, 33, intentionally set two fires six hours apart Sept. 11 at a residence in the 1300 block of Herman Street, according to a criminal complaint.

Investigators said Travis set the fires using combustible materials and a gas can filled with flammable liquid was discovered at the first fire, court records show.

The victim and Travis had a disagreement at the victim’s home shortly before the first fire erupted, court records show.

The victim’s daughter told investigators she said saw Travis coming from the area where the first fire was set about 10 minutes before it was discovered, according to court records.

Investigators said the second fire ignited six hours later in the same location, according to court records.

Authorities said a neighbor smelled smoke, went outside and discovered the second fire, court records show.

Authorities said the same neighbor saw Travis’ vehicle leave the scene, and the neighbor tried to extinguish the second fire with his garden hose, according to court records.

Prosecutors recommended Travis’ bond be set at $10,000, court records show.

He was not listed in Potter County jail records on Friday.

Wrongful Death 20 Yr Old Ohio Woman Pleads Guilty

(CBS) CINCINNATI – An Ohio woman pleaded guilty Wednesday to escape the death penalty for the gruesome killing of 29-year-old Celeste Fronsman, who was found badly burned and wailing in agony on the side of a rural road.

Katrina Culberson, 20, pleaded guilty to one count each of aggravated murder, kidnapping and aggravated arson as part of a deal with prosecutors. Muskingum County Prosecutor Mike Haddox said his office offered the agreement to Culberson in exchange for her testimony against two others charged in the killing of Fronsman.

“She understood all the terms and conditions,” Haddox said. “She entered into it knowingly and willingly.”

Prosecutors can cancel the agreement if Culberson becomes uncooperative or unwilling to testify against LaFonse Dixon and Monica Washington, Haddox said.

A driver found Fronsman on Aug. 26 on a road near Zanesville in eastern Ohio. She was raped, burned and had a strap around her neck. She died two days later at a Columbus hospital.

In a jailhouse interview with the Times Recorder in Zanesville this month, Culberson said she, Washington and Dixon “all had a part” in the killing.

“I want to be a human being again, not some monster,” she said. “I want to ask God for forgiveness for everything I’ve ever done.”

Both Dixon and Washington have pleaded not guilty to aggravated murder, kidnapping and aggravated arson. They face a potential death sentence if convicted.

Court records show that Fronsman was involved in prostitution and she was arrested more than 20 times since 2003, mostly for domestic violence, cocaine possession and prostitution.

Fronsman’s family has said the young woman was a good person with a good heart who was devastated by the death of her 2-year-old daughter in 2005 and her mother in 2009.

Accident Reconstruction Fatal Crash on I-64 in Carter County, KY

Kentucky State Police say wet roads were a factor in a fatal crash on eastbound Interstate 64 in Carter County at 2:30 pm Friday afternoon.

Both lanes at milemarker 159 were closed for hours due to the crash involving a tractor-trailer and a red car.

Kentucky State Police say Mary R. Logan, 53, of Lewis County, the sole person in the red car, hydroplaned in the rainstorm. She struck the guardrail and then was hit by a tractor-trailer. Logan was pronounced dead at the scene. Police say she was not wearing a seatbelt.

To avoid the collision, police say it appears the tractor-trailer hauling beauty products bound for South Carolina swerved out of the way, crashing through the guardrail and down the embankment, landing at a sharp angle. Police say the man and woman inside the tractor-trailer were fine, but were taken to the hospital to be checked out.

Carl Johnson works for Bubba’s Towing, a local company. As soon as he saw the scene, he knew who had been in the red car.

“Everybody in the local area knows everybody, knows family of each other, so when you recognize last names or vehicles it’s rough. But in our profession, what we do for a living, it’s something we learn to deal with,” says Johnson.

Johnson says Mary Logan is a mother, and his thoughts are with her family.

The long task of clearing the interstate took several hours. The Environmental Protection Team had to remove the diesel from the tractor-trailer, and then remove a stretch of guardrail, before they could begin to tow the tractor-trailer out of the embankment.

A detour was set up at milemarker 156 on I-64 East all day. That area of the interstate was expected to be open by the early morning hours Saturday.

Fraud Investigaiton George Mathews Indicted

A Prince Frederick physician was indicted Monday by a federal grand jury on charges of illegal distribution of drugs and health care fraud in connection with pain management clinics he operated in Prince Frederick, Waldorf and Greenbelt.

George Mathews, 75, was indicted on 14 charges of distributing and dispensing oxycodone, eight charges of distributing and dispensing methadone and 15 counts of health care fraud.

As a medical doctor, Mathews, according to the indictment, was authorized to dispense medicine, including controlled dangerous substances, to patients for legitimate medical purposes. The indictment alleges that Mathews ran a “pill mill” out of his offices at which individuals could obtain prescriptions for controlled substances, including oxycodone and methadone, for a fee and without any legitimate medical purpose, according to a U.S. District Court of Maryland press release.

The indictment states that from at least 2007 through July 2011, Mathews filed fraudulent insurance claims in a scheme to defraud the Office of Workers’ Compensation Programs of the U.S. Department of Labor, the U.S. Postal Service, Medicare, Medicaid and other public and private insurance carriers, according to the press release. Mathews allegedly submitted claims falsely seeking reimbursement for patients’ office visits that never occurred, including billing for an office visit at a time that Mathews had been admitted to the hospital for his own health problems.

Mathews also allegedly fraudulently double billed for his services, collecting a cash payment from a patient for an office visit and later improperly filing a claim for those same services. Mathews, the press release states, also allegedly “upcoded” many of his patients’ office visits, claiming that he had met with the patient for a much longer period of time than he actually had, in order to obtain a higher reimbursement. The indictment states that on more than 250 occasions, Mathews sought reimbursement for services he allegedly rendered in excess of 24 hours in a day.

The indictment seeks forfeiture of at least $3,190,899, which is believed to be the proceeds from the health care fraud scheme. Mathews faces a maximum sentence of 20 years in prison for each count of illegal distribution of drugs and a maximum of 10 years in prison for each count of health care fraud, the press release states.

The indictment was announced by U.S. Attorney for the District of Maryland Rod J. Rosenstein; Special Agent in Charge Ava Cooper-Davis of the Drug Enforcement Administration, Washington Field Division; Special Agent in Charge Joanne Yarbrough of the U.S. Postal Service, Office of Inspector General, Major Fraud Investigations Division; and Special Agent in Charge Michael S. Barcus, U.S. Department of Labor Office of Inspector General, Office of Labor Racketeering and Fraud Investigations.

“This indictment marks a significant effort in the ongoing investigation against fraudulent workers’ compensation claims,” Yarbrough said in a written statement. “The workers’ compensation program benefits thousands of postal employees who have received legitimate on-the-job injuries. But false claims by health care providers undermine the system. We appreciate the partnership of the DEA, the U. S. Department of Labor-OIG, and the U.S. Attorney’s Office, District of Maryland on this case.”
