Missing Person Brian McCamish of Big Bear Lake, CA

The Big Bear Sheriff’s Station is asking for the public’s help in locating a Big Bear Lake resident.

Sixty-year-old Brian McCamish is described as a “voluntary missing person” and was last seen in the City of Big Bear Lake on September 19. It’s not known if he’s currently in the area, but anyone who has information on his whereabouts is asked to report it to the Sheriff’s Station at (909) 866-0100.

McCamish is a 165-lb., 6’3″ white male, with short blond hair, blue eyes, and burn scars on his face. He may be wearing prescription eyeglasses with dark frames.

Wrongful Death FBI Investigates Beheading in Moore, OK

A man beheaded a woman at an Oklahoma warehouse and attacked another woman before being shot, police say.

The incident happened late Thursday afternoon at a Vaughan Foods processing plant in Moore, Okla., about 10 miles south of Oklahoma City. According to KFOR, Alton Nolen, 30, had just been fired when he drove to the front of the business, hit another vehicle and walked inside. He tracked down 54-year-old Colleen Hufford and attacked her with a knife.

He stabbed her several times, eventually severing her head, KFOR reported, citing police. He then sought out another woman–identified as 43-year-old Traci Johnson–and began attacking her. Mark Vaughan, an Oklahoma County reserve deputy and a former CEO of the business, shot Nolen, who’s in stable condition at a hospital.

The FBI is now involved in the investigation. Federal agents are looking into claims from workers that Nolen had recently converted to Islam and tried to get co-workers to join him.

Private Detective: Man Shot Twice on Charles Ave. in Lexington

Police are investigating a shooting on Charles Avenue in Lexington, where one man was shot twice. It happened at around 10 p.m. on Friday near the intersection of Hickory Street.

Police said the victim was taken to a nearby hospital with non-life threatening injuries.

Officers said they are searching for a black passenger car that may have been involved in the shooting.

Investigators said the victim was driving a white Impala, that officers later found up the road near Whitney Avenue.

Several different neighbors told LEX 18 News a similar story, that they heard eight or nine gun shots, a pause and then two more gun shots. Our crews counted around nine shell casing markers on scene.

Police spent more than two hours at the scene and said they are investigating whether or not this case was related to an early shooting on Bainbridge Court.

Missing Person Pulaski County, AR Realtor Missing After Showing House

UPDATE: (4:50 p.m.):

The sheriff’s office is asking any farmers or owners of large areas of land in the Scott area to please check their properties to help in the search for Beverly Carter.

Police are using a helicopter and search dogs to look for Carter. Volunteers and fellow realtors are also helping search and hand out flyers.

ORIGINAL REPORT (10:30 a.m.):

Pulaski County Sheriff’s Office is searching for missing realtor Beverly Carter.

Authorities were called to 14202 Old River Drive around 11:00 Thursday night after reports of a missing person. Officers said they met Beverly’s husband Carl Carter in the driveway of the residence.

Mr. Carter told police he had not been in contact with his wife since she had last called him around 5:30 p.m. Beverly Carter works as a realtor for Crye-Leike Realty. She told her husband she was going to show the house at 14202 Old River Drive to an unknown person. Around 9 p.m. he became worried when he had not heard from his wife and went to check on her at the home.

Mr. Carter told police he found her brown 2014 Cadillac SRX with the license plate “BCARTER” parked in the driveway with her purse inside. He also found the door to the residence open, so he walked inside and searched for his wife but did not find her.

Beverly Carter was last seen wearing a black sleeveless shirt and red shorts.

Wrongful Death Trial Date Set For Clark Co. Man Accused

A trial date has been set in the 2013 death of Kyla Kline, a Clark County woman.

Kline’s body was found under a pile of brush and logs on a Montgomery County farm.

Martin faces another charge for the 1995 murder of his cousin, Joseph Martin. During questioning, Lonnie Martin admitted to pushing Joseph in front of a train in Clark County.

The Winchester Sun reports that Lonnie Martin will head to court May 4, 2015.

Fraud Investigation IRS Telephone Scam Alert

The Internet Crime Complaint Center (IC3) has received reports related to a telephone scam in which the caller purports to be an Internal Revenue Service (IRS) representative. Using intimidation tactics, the caller tries to take control of the situation from the beginning. The caller advises the recipient of the call that the IRS has charges against them and threatens legal action and arrest. If the recipient questions the caller in any way, the caller becomes more aggressive.

The caller continues to intimidate by threatening to confiscate the recipient’s property, freeze bank accounts, and have the recipient arrested and placed in jail. The reported alleged charges include defrauding the government, money owed for back taxes, law suits pending against the recipient, and nonpayment of taxes.

The recipients are advised that it will cost thousands of dollars in fees/court costs to resolve this matter. The caller creates a sense of urgency by saying that being arrested can be avoided and fees reduced if the recipient purchases moneypak cards to cover the fees within an hour.

Sometimes the caller provides specific instructions on where to purchase the moneypak cards and the amount to put on each card. The caller tells the recipient not to tell anyone about the issue and to remain on the telephone until the moneypak cards are purchased and the moneypak codes are provided to the caller. The caller states that if the call is disconnected for any reason, the recipient would be arrested. Some recipients reported once the caller obtained the moneypak codes, they were advised that the transaction took too long and additional fees were required.

Call recipients, who are primarily immigrants, reported that the caller spoke with broken English or stated the caller had an Indian accent.

  • Resist the pressure to act quickly
  • Report the contact to TIGTA at http://www.treasury.gov/tigta by clicking on the red button, “IRS Impersonation Scam Reporting.”
  • Use caution when asked to use a specific payment method. The IRS would not require a specific payment method such as a moneypak card or wire transfer
  • If you feel threatened, contact your local police department

Wrongful Death Physician Charged With Prescribing Meds That Resulted in The Death of a Patient

A Louisville physician was charged today, by a federal grand jury, with prescribing medications that resulted in the death of a patient, as well as multiple counts of unlawful distribution of controlled substances, health care fraud and money laundering announced David J. Hale, United States Attorney for the Western District of Kentucky.

According to the superseding indictment, George Kudmani, age 69, between July 29, 2009 and May 3, 2010, executed a scheme to defraud Medicare that ultimately resulted in a patient’s death on May 5, 2010. According to the charge, Kudmani fraudulently submitted claims for medically unnecessary services and dispensed medically unnecessary controlled substance prescriptions to the patient, in exchange for money, knowing that the patient would fill her prescriptions at pharmacies and pharmacies in turn would submit claims to health care programs for reimbursement. It’s alleged that those prescriptions ultimately resulted in the patient’s death.

Further, defendant Kudmani is charged with a second count of health care fraud for falsely and fraudulently billing Kentucky Medicaid (Passport) and other medical benefit programs by submitting claims for medically unnecessary Transvaginal Ultrasounds (TVS), TVSs not performed, and billing for TVS reports that were never prepared for patients, between January 2009 through September 2012.

The superseding indictment charges Kudmani with eleven additional counts, (for a total of 22) of unlawful distribution and intentionally distributing and dispensing controlled substances, not for a legitimate medical purpose and beyond the bounds of a professional medical practice, between July 2009 and September 2012. The controlled substances allegedly prescribed were Oxycodone, a schedule II controlled substance, and Hydrocodone, a schedule III controlled substance.

In addition, Kudmani is charged with one count money laundering for purchasing a 2012 Honda Accord with $15,000 in cash and a $5,971.63 check from money derived from an unlawful activity, that is the unlawful dispensing and distribution of controlled substances and health care fraud.

Beginning in December of 1980, Kudmani operated an obstetrician/gynecological medical practice located at 9702 Stonestreet Road, in Louisville, Kentucky. The practice did not employ any other individual with medical training. A typical first-time patient would pay $75 for a gynecological exam, and each visit thereafter, the patient would typically pay $35 in cash and receive a Schedule II-V controlled substance prescription without a physical examination.

Kudmani was initially charged in a 14 count indictment on December 4, 2013. If convicted at trial, Kudmani faces a maximum potential penalty of life in prison, a fine of $14,500,000, and a five year period of supervised release.

This case is being prosecuted by Assistant United States Attorneys Joseph Ansari and Lettricea Jefferson-Webb and was investigated by the United States Drug Enforcement Administration (DEA), Federal Bureau of Investigation (FBI), Kentucky Medical Fraud Control Unit and Louisville Metro Police Department (LMPD).

The indictment of a person by a Grand Jury is an accusation only and that person is presumed innocent until and unless proven guilty.

Wrongful Death NDSU Student Killed In Minnesota

A Minnesota medical examiner on Thursday ruled the death of a North Dakota State University student who went missing last weekend a homicide.

The Ramsey County medical examiner’s office said Thursday it had determined that Thomas Bearson, 18, of Sartell, Minnesota, died as the “result of homicidal violence.”

Bearson’s remains were found Tuesday outside an RV store in south Moorhead. He was last seen at a party early Saturday in Fargo, North Dakota. Police in Moorhead said Bearson was wearing a blue baseball cap, blue Sartell High School track and field shirt and khaki shorts.

It’s unclear whether law enforcement authorities have identified a person of interest. Shortly after the body was found, Fargo Police Chief Keith Ternes said police weren’t looking for any suspects. The investigation into Bearson’s death is a collaboration between the Moorhead and Fargo police departments and the NDSU campus police.

Authorities are asking for the public’s help to locate Bearson’s cellphone and a shoe. Police described the missing items as a white Nike Air Jordan left-foot tennis shoe, size 9 ½, and a silver iPhone 5.

Bearson, who was enrolled in the university’s nursing program, was living in a residence hall on campus before his disappearance. He graduated earlier this year from Sartell High School, where he was a standout basketball player.

Sartell athletic director John Ross heard the news while at a school athletic event Thursday night when a reporter called, the St. Cloud Times reported.

“This is going to be tough on the kids,” Ross told the newspaper.

Missing Person Jomasa Pereyra of Dallas

Dallas police are asking for the public’s help finding an 86-year-old woman last seen in East Oak Cliff.

Jomasa Pereyra walked away from the 2500 block of East 11th Street, between Cedar Crest Boulevard and Corinth Street Road, about 3 p.m.

Pereyra is described as a 5-foot-3 Hispanic woman weighing 160 pounds with brown eyes and grey hair in a ponytail. She was wearing a gray or black shirt, gray or black belt, black pants and pink and white tennis shoes. She also walks with a cane.

Anyone who sees Pereyra or knows her whereabouts should call 911 or the Dallas Police Department’s Missing Person’s Unit at 214-671-4268.
