Missing Person Swen Carlson of Beaverton, Oregon

On April 30th, 2014 Beaverton Police Department took a report that 21 year old Swen Carlson was missing. Carlson walked away from his job in Portland during his lunch break on Apr il 29th, 2014. On April 30th, 2014 Carlson apparently went to the apartment he was staying at located in the 10000 block of SW 5th Street Beaverton, Oregon. Carlson changed his clothes, opened his mail and left without being seen. Carlson’s roommates and family have not seen or heard from him since he went missing. Carlson may have some mental health issues that have not been diagnosed.

Private Detective, Missouri Man Indicted on Federal Bank Robbery Chares

Joshua Kelly was indicted in connection with the April 16, 2014 armed robbery of the First Bank in Montgomery County.

Kelly, 28, of New Florence, Missouri, was indicted by a federal grand jury on one felony count of bank robbery and one felony count of possession of a firearm in furtherance of a crime of violence.

If Kelly is convicted, bank robbery carries a maximum penalty of 20 years in prison and/or a fine of up to $250,000. The firearm charge carries a penalty range of five years to life in prison. In determining the actual sentences, a judge is required to consider the U.S. Sentencing Guidelines, which provide recommended sentencing ranges.

This case was investigated by the Kirksville Resident Agency of the Federal Bureau of Investigation; the Lexington, Kentucky Resident Agency of the FBI; and the Montgomery City Police Department. Assistant United States Attorney Cristian M. Stevens is handling the case for the U.S. Attorney’s Office.

As is always the case, charges set forth in an indictment are merely accusations and do not constitute proof of guilt. Every defendant is presumed to be innocent unless and until proven guilty.

Private Detective, Inmate Escapes Work Release in Woodford County

Officials are looking for an inmate they say escaped during a work release in Woodford County Thursday morning.

Police say state inmate Andrew Reeves was on work release on Crossfield Drive around 11:30 a.m. when he jumped a fence and took off by the railroad tracks in the area. Police say he took off his orange vest and started running.

A perimeter has been set up and officials are focusing on the area near railroad the tracks. Woodford County High School, which is located near the tracks, is on lockdown.

Reeves is described as a white male last seen wearing black t-shirt and blue jeans.

Woodford County Sheriff’s Department, Versailles Police and UK Police are assisting in the search.

Private Detective, Human Foot in Sneaker Found Near Seattle’s Pier 68

A Port of Seattle spokesman says a human foot in a white sneaker has been found on the shore at a park near Seattle’s Pier 68.

The Seattle Times reports that port spokesman Peter McGraw says volunteers cleaning the park found the shoe and foot Tuesday. Port police turned both over to the King County medical examiner’s office.

To help identify the person whose foot was in the shoe, the King County Medical Examiner’s Office released a photo of the shoe. The shoe is a New Balance athletic sneaker, men’s size 10-1/2, white with blue trim. This model of shoe was first available for sale in April 2008.  A black, cotton Hanes brand sock was also on the foot.  The sex, age and ancestry of the person are not known.

Anyone who knows of a missing person that may have worn this type of shoe is asked to call the King County Medical Examiner’s Office at 206-731-3232, ext. 1. Also, anyone aware of a missing person who has not been reported is asked to file a missing persons report with the appropriate local law enforcement agency.

Since 2007, there have been roughly a dozen cases in which a human foot encased in a tennis shoe has washed ashore in regional waters.  A number of them were found in British Columbia. In a couple of cases, investigators determined the feet belonged to men who had been reported missing.

The Times reports that oceanographer Curtis Ebbesmeyer of Seattle says it’s common for decomposing bodies to come apart at the joints, including at the ankles. He knows of at least two feet turning up in Puget Sound over the past decade. He says new lightweight sneakers stay buoyant and float sole up, protecting the remains.

Missing Person Camden, MS Toddler Still Missing, $20k Reward

The search for a Madison County toddler who has now been missing two months is ongoing, according to officials.

Myra Lewis, 2, went missing from her Camden home on March 1, which generated a massive manhunt in the woods surrounding her home. Her mother, Ericka Lewis, said the last time she saw the child she was in front of their home with her siblings. Her father, Gregory Lewis, was in the back of the home with an infant and didn’t realize she was missing for several hours.

The land around the home, including a lake across the street and some heavily wooded areas, were searched by specialized search teams, dogs, law enforcement and volunteers, and turned up nothing.

FBI Spokeswoman Deborah Madden said authorities are still hard at work on the case.

“We’re continuing to follow every lead,” Madden said. “We encourage the public to call the FBI or the Madison County Sheriff’s Office if they have any information.”

Madden said the $20,000 reward offered by the FBI for tips leading to the location of the toddler is still on the table for anyone who will step forward with the information.

Investigators have said it’s almost as if Myra disappeared into thin air. The investigation has stymied authorities, who have searched the land around her house and followed numerous leads since her disappearance.

At one point an SUV was taken into police custody on a tip that the child had been run over, but officials said there was no blood inside of it. So far it is not clear if there was evidence on the outside of it.

Ericka Lewis was jailed on probation violations when she was arrested for being a felon in possession of a firearm shortly after the child went missing. Investigators had located some guns inside the family’s home. Lewis’ probation violation stemmed from a felony food stamp fraud conviction. Lewis’ hearing was delayed until May 19 because the sentencing judge was unable to be in court.

Family members have said they believe the child was abducted. Madison County Sheriff Randy Tucker said his hope is that Myra will be found alive, but authorities also contend disappearance does not seem typical of a random kidnapping.

Ericka Lewis’ mother, Martha Sanders, said she feels like the arrest was a tactic to pressure her daughter into giving up some kind of information.

“They can pressure Erica, but she’ll never give in. She’ll never give in because she didn’t do anything,” said Sanders a few days after Ericka Lewis’ arrest.

Myra’s family has been polygraphed, but police won’t comment on the outcome.

When asked about the latest on the investigation, Madison County Sheriff’s Department spokesman Heath Hall said, “We do not have any updates today.”

Missing Person Mario Frias of Chaparral, NM

State Police have issued an endangered missing person alert for Chaparral resident Mario Frias.

Frias is disabled and requires medication daily, according to a State Police news release.

Frias was last seen May 4 in Chaparral. He is 30 years old and 5 feet 10 inches tall. He has brown eyes and black hair and weighs 190 pounds.

He has a tattoo of spinning stars on his upper left arm, as well as pierced ears and multiple surgery scars on his head and knee, according to the release.

Missing Person Body of Ohio County Man Found

The Ohio County Sheriff’s Office said they have found the body of a man who had been missing for nearly a week.

Deputies said Albert Dockery, also known as “Birdo,” had presumably walked away from his home on Highway 62 outside of Horse Branch May 1.

Search teams found the 68-year-old’s body Wednesday morning in a creek near his home.

Officials said Dockery’s family believes he was in the early stages of dementia.

Missing Person NJ Man Last Seen Renting a car at BWI-Marshall Airport

A missing New Jersey man was last seen renting a car at BWI-Marshall Airport. That car was found abandoned in Calvert County.

Hani Heibi, 29, from Springfield, N.J. has been missing since Sunday.

Police say he routinely travels from New Jersey to St. Mary’s County, Md. for work.

Heibi left New Jersey on Sunday, traveling to Maryland by train and then renting a car at the Avis Rental Car office at BWI around 11:30 p.m.

The rental vehicle which police believe Heibi was operating is a grey Ford Fusion, which was found abandoned at a hotel in Prince Frederick.

Heibi is described as a white man, 5’11″ tall, 170 lbs, with brown hair and brown eyes.  He was last seen wearing a white shirt and a dark colored jacket.

An extensive search for Heibi was completed throughout the Prince Frederick area. Local surrounding businesses, hotels and Calvert Memorial Hospital were also checked. Preliminary interviews were completed with family members.

Insurance Fraud Newport Chiropractor Convicted of Health Care Fraud Conspiracy

The owner of a chiropractic clinic in Newport, Kentucky, has been convicted of a fraud scheme involving tens of thousands of dollars.

A federal jury in Covington convicted 48-year-old Andrea Almond, of Cincinnati, Ohio, of conspiracy to commit health care fraud, late Friday afternoon. The jury returned the verdict after four days of trial, following six hours of deliberation.

According to evidence established at trial, from September 2008 through April 2010, Almond conspired with a co-defendant to bill the Kentucky Medicaid Program for chiropractic services rendered at the Newport Chiropractic Center.

The evidence, however, also established that Almond never actually practiced at the Newport Chiropractic Center; the billed services were instead provided by other doctors, who had never been approved or enrolled in Kentucky Medicaid Program.

Almond billed the Kentucky Medicaid Program using her National Provider Identification number, knowing that the Kentucky Medicaid Program would not have paid for any claims for treatment rendered by unapproved providers.

The government proved that Almond and her co-conspirator fraudulently submitted approximately 1,943 claims to the Kentucky Medicaid Program, which totaled $483,797, and they received $95,098 in payments.

Her co-conspirator, Elaine Legg, has already pled guilty in the case. In a separate case, Dr. Nanci Allen, a former part owner in the Erlanger Chiropractic Clinic, has pled guilty to a similar scheme to defraud the Kentucky Medicaid Program. These individuals are also awaiting sentencing.

Kerry B. Harvey, United States Attorney for the Eastern District of Kentucky; Perrye Turner, Special Agent in Charge, Federal Bureau of Investigation; Dugan Wong, Postal Inspector in Charge, United States Postal Inspection Service, Pittsburg Field Division; and Jack Conway, Kentucky Attorney General, jointly made the announcement today.

The investigation was conducted by the Federal Bureau of Investigation; the United States Postal Inspection Service, Cincinnati Field Office; the Kentucky Attorney General’s Office, Medicaid Fraud Abuse and Control Unit; and the Kentucky Department for Medicaid Services. The federal government was represented by Assistant United States Attorney Laura K. Voorhees.

Almond will appear for sentencing on September 4, 2014. She faces a maximum of 10 years in prison. However, any sentence following a conviction will be imposed after the court considers the U.S. Sentencing Guidelines and the applicable federal statutes.
