Missing Person Irma Mkrtchyan of Las Vegas, Nevada

Metro Police are searching for a 45-year-old woman who has been missing since late January.

Irma Mkrtchyan disappeared Jan. 19 and was last seen in the area of West Desert Inn Road and South Durango Drive. She is described as a 5-foot-5, 120-pound white woman with blonde hair and brown eyes.

Accident Reconstruction 2 Killed and One Arrested in Madison County 5 Vehicle Crash

 

Two people have died, a third person was hurt and another person is charged with aggravated drunken driving after a five-vehicle wreck on Interstate 75 in Madison County Sunday.

 

Kentucky State Police said 22-year-old Meredith L. Buscher of Richmond and 30-year-old Jeffrey Campbell of Louisville died after being hit by a car on I-75. Police said the two had pulled over to help people involved in another accident when they were struck by a vehicle trying to avoid the car that was blocking the road.

KSP said the multi-vehicle collision happened around 7:30 p.m. in the northbound lanes of I-75 near mile marker 92 when a white 1999 Ford Explorer operated by Bryan M. Mangan lost control colliding with a guardrail and other vehicles, and overturned, partially blocking the left lane of the interstate.

 

Police arrested 56-year-old Mangan of South Bend, Ind., and charged with driving on a suspended license and aggravated drunken driving. He was taken to the Madison County jail.

 

According to his citation, Mangan told officers he drank a few beers.

 

Jeffrey Campbell, was pronounced dead at the scene by the Madison County Coroner. Meredith Buscher was trasported to the University of Kentucky Hospital where she died as a result of her injuries.

 

Another person was hurt, 41-year-old Clifford H. Steinhauer of Thomas, Ky., who was transported to the University of Kentucky Hospital for treatment.

 

Mangan’s ex-wife in Indiana told LEX 18 Monday morning she briefly spoke to Mangan from the hospital Sunday night where he told her he was still in shock from what happened.

 

The collision remains under investigation, KSP said.

Wrongful Death More Details On Death of Lexington Baby

More details have been released in the death of a 29-day-old baby who was taken to the hospital after becoming unresponsive at home on Thanksgiving of last year.

The Fayette County Coroner said Monday the child died of blunt force injuries of the head, and the death is being investigated as a homicide.

Caz Hazuga, II, the baby’s father, is accused of killing the infant. Hazuga claims he didn’t kill his son. He said he woke up to feed little Caz Hazuga, Jr. at 6 a.m., and the baby choked on his formula and stopped breathing.

The autopsy was complete on Nov. 29, 2013. The information was not released to the media until Monday.

Wrongful Death Man Shot and Killed in Lexington

Officials have identified the man shot and killed inside a car along Lima Drive Sunday afternoon.

The Fayette County Coroner said Vincenzo Happy, 23, died from a gunshot wound at UK Hospital.

Police said they found Happy in the car, badly wounded around 2 p.m. Sunday. Officers located 10-12 shell casings on the ground. They said they believe the car came to a complete stop after hitting a snow bank.

Investigators said surveillance video from surrounding businesses shows several people in the parking lot at the time of the shooting. Police said they hope to get ahold of those witnesses to find out exactly what happened.

Fraud Investigation Addiction Treatment Center To Pay $15.75M For Fraudulent Urine Drug Testing

A chain of opiate addiction recovery centers, headquartered in Harrodsburg, and a Russell Springs, Ky., clinical laboratory, along with two physician owners, agreed to pay the U.S. Government millions of dollars to resolve civil allegations that they fraudulently billed federal health care programs for medically unnecessary and excessive urine tests.

PremierTox 2.0, LLC (“PremierTox”), Addixxion Recovery of Kentucky, LLC d/b/a SelfRefind (“SelfRefind”), Dr. Bryan Wood and Dr. Robin Peavler have agreed to pay a total of $15,750,000, plus interest, to resolve allegations that they violated the False Claims Act by submitting claims to Medicare and Kentucky’s Medicaid program for urine tests that were medically unnecessary and more expensive than the actual tests that were performed.

Under federal law, health care programs only reimburse health care providers for services that are deemed medically necessary.

“Federal health care programs are essential to many of our citizens,” said U.S. Attorney Kerry B. Harvey. “We will not tolerate efforts by misguided providers to unfairly enrich themselves at the expense of these programs and the taxpaying public.

This settlement underscores the continuing commitment of our office to use every available tool to protect these vital programs from false claims.”

According to the settlement agreement, Dr. Wood and Dr. Peavler owned and operated SelfRefind, a chain of addiction treatment clinics located in 12 Kentucky cities including, Danville, Frankfort, Hazard, Middlesboro, Pikeville, Barbourville, Morehead and Carrollton. As part of its treatment program, SelfRefind required all of its patients to submit to regular urine drug screening, as often as every two weeks, to ensure that the patients were not abusing controlled substances and were taking addiction treatment medications as prescribed.

The government alleged that after Wood and Peavler became owners of PremierTox they began automatically referring all drug screens completed at SelfRefind to PremierTox clinic for additional comprehensive urine drug screening tests that were frequently unnecessary and often more expensive than suitable alternative tests. The government also alleged that PremierTox submitted false claims that misidentified the class of drug that was tested for and received a higher financial reimbursement than necessary.

Before Dr. Wood and Dr. Peavler became part owners of PremierTox, SelfRefind did not automatically refer urine samples for additional confirmation testing to outside laboratories. According to the settlement agreement, in December 2010 when Drs. Wood and Peavler referred urine samples to PremierTox, the lab did not have the equipment necessary to test the large volume of urine samples sent to it by SelfRefind. Therefore, PremierTox froze the samples in a storage unit for many months before performing the additional tests, which by that time were medically unnecessary for the treatment of the patients. Nevertheless, PremierTox submitted claims, seeking reimbursement for the tests.

In connection with the settlement, PremierTox has agreed to enter into a Corporate Integrity Agreement with the Department of Health and Human Services, Office of Inspector General (“HHS-OIG”). The agreement obligates PremierTox to undertake substantial internal compliance reforms and commit to a third-party review of its claims to federal health care programs for the next five years. The agreement also resolves allegations that the defendants violated the Stark Law, which forbids a laboratory from billing Medicare and Medicaid for certain services referred by physicians that have a financial relationship with the laboratory.

“Billing Medicare and Medicaid for laboratory tests that are not necessary contributes to the soaring costs of health care,” said Assistant Attorney General for the Civil Division Stuart F. Delery. “Providers will be aggressively investigated and held accountable for falsely billing federal health care programs.”

The Commonwealth of Kentucky is also a party to the agreement and will receive approximately $2.74 million, which represents the state’s share of the government’s recovery of Medicaid funds. The Medicaid program is funded jointly by the federal and state governments.

Accident Reconstruction Two Dead in Madison County Accident

Two people have died and at least one other person was injured in a four-vehicle crash on Interstate 75 near mile marker 91 in Madison County Sunday evening.

Police said one person died at the scene, and a second victim died at the hospital.

Police said an initial crash happened and two people went to the wrecked car to help the victim. While they were standing outside the car, a second car drove by and struck them.

One southbound lane and the two left lanes northbound were closed for hours while reconstruction teams worked to determine what caused the accident. Due to the snow, crews decided to re-open the road around 10:30 p.m. and come back to the scene Monday morning.

Police haven’t released which hospital victims were sent to or their names.

Missing Person Nancy Kilerciyan of New York

The Village of Piermont Police Department is investigating a missing person case involving an at risk woman.

Nancy Kilerciyan, 58, was last seen in the rear parking lot of Cornettas Restaurant at 6 p.m. on February 8, her family said.

Kilerciyan is on medication and is considered at risk, she was last seen wearing blue jeans and a red jacket.

Missing Person Lafayette, CO Man Still Missing

It has been nearly three weeks since Lafayette resident Nathaniel Tallman went missing, and a $10,000 reward offered up by his family and friends has not yet helped elicit any information about his whereabouts, authorities say.

Tallman, 25, was last seen at about 6 p.m. Jan. 21 near the intersection of Empire Road and U.S. 287 in Lafayette, friends say.

He was driving his 2014 Mitsubishi Lancer Rally with temporary tags.

Friends say Tallman was on his way to Longmont. He never arrived and has not been seen or heard from since.

Information about his whereabouts or that of his car also have not surfaced, despite family and friends announcing on Jan. 29 that they were offering a $10,000 reward for information useful to their search for the 2006 Monarch High School graduate.

“We’re not doing very well,” Tallman’s father Kevin Tallman said last week of the state of the family in the wake of his son’s disappearance. “We’re just trying to live every day waiting for good news.”

He urged people to share any information they may have and asked that people continue to look for his son.

“I would appreciate everyone’s help in looking for my son and his car,” he said. “That $10,000, I’d be happy to give that to somebody if they could help us find our boy. It’s a small price to pay.”

Lafayette police entered Tallman into the department’s computer system as a missing person within days of his last being seen. Investigators have notified other state law enforcement agencies about Tallman’s disappearance and have been following tips related to the case for weeks, according to the Lafayette Cmdr. Brain Rosipajla, but have no new information about where he or his car may be.

“The status of this case is still under investigation,” Rosipajla said. “We are asking the community if they saw anything to please call the Lafayette tip line. We are following up on all the tips we get.”

Nathaniel Tallman is described as white, with brown hair and hazel eyes, standing about 5-foot-10-inches tall. He was last seen wearing black sweatpants with a white stripe down the side and a white North Face jacket.

Friends and family told police they do not believe he intentionally ran away or took his own life.

Pedophile Tracking Nebraska Man Sentenced for Attempted Assault

A Douglas County judge has sentenced a South Sioux City man to 90-days in jail and probation for his conviction of attempted sexual assault of a child in Valley, just west of Omaha.

Sioux City, Iowa television station KTIV reports that 48-year-old Marc Mallory pleaded no contest Friday to the charge in Douglas County District Court and received credit for 21-days already spent in jail.

Once he’s released, he’ll serve five years of probation and must register as a sex offender.

Police say Mallory entered the bedroom of a sleeping 11-year-old girl and fondled her.

The judge also ordered Mallory to have no contact with any female under the age of 18.
