Police have issued a missing person alert for a 16-year-old girl who was last seen a week ago in the Northwest Side Albany Park neighborhood.

Shivani Patel was last seen on July 20 in the area of Lawrence and Springfield, the alert said. Her current whereabouts, or whom she may be with, were unknown.

Missing Person Darcy Miller of Orlando

A 30-year-old Orlando woman vanished this week while heading to The Mall at Millenia, prompting a missing-person alert from the Orange County Sheriff’s Office.

Darcy Arlene Miller called family members at about 10:15 a.m. Thursday to say she had left her condominium and was driving to the mall in her Honda Fit.

That was their last contact with Miller, who lives at Lakeside at the Lakes of Windermere Condos and needs certain medications for an undisclosed medical condition.

A photo of Miller released by deputies shows her smiling while holding a hedgehog to her left shoulder.

“There is concern for her well-being, as it is unusual behavior for Darcy to not keep in contact with her family or friends. Darcy is [on] prescribed medication that she does not have with her,” the Sheriff’s Office said in an alert Friday.

“Her disappearance for more than day, and the much needed medication that she is without, has her family and friends extremely worried. If you see Ms. Miller or her vehicle please call one of the numbers listed on the bulletin or 911,” the alert added.

Miller was driving a white 2007 Honda Fit with this Florida tag: I489FN. The car has two white, cat-shaped stickers on the back glass on the driver’s side of the vehicle.

“Ms. Miller has throughout her life never disappeared without letting someone know unless it was for a very short period of time,” the alert said.

Missing Person Remains Found in Septic Tank in St. Francois County, MO

Authorities say the remains of a missing person were discovered in a septic tank in St. Francois County on Thursday.

The St. Francois County Sheriff’s Department said the badly decomposed remains were found on unoccupied property on Highway K, located just northeast of Bonne Terre around 12 p.m.

Neighbors told News 4 the tank was located behind a garage next to a cabin where no resides. They said investigators had been at the property since Wednesday morning.

Deputies did not confirm the victim’s identity. An autopsy was scheduled for Friday morning.

Authorities said the body’s discovery was the result of a seven-month missing person investigation. Additional information was not immediately available.

Bonne Terre is located about 50 miles southwest of St. Louis.

Missing Person Duncan James Templer of Hillsboro, ND

The McKenzie County Sheriff’s Department and assisting agencies were searching Friday for a Hillsboro, N.D., man who told a family member he planned to commit suicide in the Badlands.

Authorities are looking for Duncan James Templer since Wednesday on foot and using all-terrain vehicles and dogs, the Bismarck Tribune reported.

Templer, 51, is a truck driver for Trans Systems of Great Falls, Mont., and the search has covered a radius from his truck found unlocked and empty on a U.S. Highway 85 overlook south of the Theodore Roosevelt National Park North Unit.

McKenzie County Sheriff John Fulwider said he is treating Templer as a missing person and said it’s possible the man left his truck and is with someone else.

The sheriff’s department asks anyone with information to call (701) 444-3654.

Wrongful Death Greenup County Death UPDATE

Police now have more details about the murder of a northeastern Kentucky woman.

Investigators say an autopsy shows that Mary Dixon, whose body was found in a Greenup County creek, was beaten and stabbed to death.

Thursday, her accused killer, Arthur Quigley, pleaded not guilty in court.

The autopsy say his alleged victim was stabbed in the neck, and had injuries to her head.

Dizon was last seen alive on June 26th. Her body was found three days later.

Identity Theft Jackson County Woman Sentenced to Over 36 Years

A Jackson, Ky., woman, who previously admitted to stealing someone else’s identity to buy a car, was sentenced to 39 months in federal prison.

On Thursday, U.S. District Judge Karen Caldwell sentenced 45-year old Lisa Ann Salyers for wire fraud and aggravated identity theft. Judge Caldwell also ordered that Salyers pay $5,450.50 in restitution to Paul Miller Ford.

Salyers previously admitted she obtained the date of birth and social security number of another person through the internet. She then assumed the identity of this victim and used it to purchase a car from a Lexington car dealership. Salyers also tried to use the victim’s identity to buy furniture from a Lexington retailer.

Salyers pleaded guilty to these charges in March of 2013.

Under federal law, Salyers must serve at least 85 percent of her prison sentence, and will be under the supervision of the U.S. Probation Office for three years following the completion of her prison term.

Wrongful Death Louisville Man Enters Guilty Plea

The Louisville stepfather who admitted to beating his stepson to death will spend the rest of his life in prison.

Josh Gouker entered a guilty plea in the May 2011 murder of 14-year-old Trey Zwicker. He said he started getting mad when he noticed Zwicker had taken his cigarette lighter and eaten a plate of food left for him. He says he invited Zwicker down to a spot at a creek behind a high school, where the two smoked marijuana, then Gouker snapped.

Gouker’s son, Joshua Young, faces a murder charge in the case. His trial is set to start Monday.

Accident Reconstruction Teen Killed in Perry County Wreck

A Perry County teen was killed and another man injured in a wreck Thursday night.

The accident happened at about 7:15 on Kentucky 15 in the Bonnyman community of Perry County.

During the preliminary investigation at the scene, it was determined that a 2011 Dodge Charger traveled off the highway, collided with a guard rail, then overturned before striking a tree. The car came to rest in a private driveway. Two occupants in the vehicle were ejected.

Both were transported to Hazard ARH before being airlifted to the University of Kentucky Medical Center. Johnathan Scott Blank, 19, of Hazard, was pronounced dead at UK medical center. Douglas Andrew Blank, 23, of Hazard is listed in critical condition.

Identity Theft 4 Suspects Charged in Maricope County, AZ Raid

Four of nine suspects arrested by the Maricopa County Sheriff’s Office in a raid last week at the Uncle Sam’s restaurant chain have been charged with identity fraud and related crimes.

The Maricopa County Attorney’s Office concluded there was insufficient evidence to charge the other five, who were turned over to federal immigration authorities, said Jerry Cobb, a spokesman for the office.

The raid was the first since a federal court ruling in May, when a judge found that the Sheriff’s Office had engaged in racial profiling against Latinos.

The four defendants were charged in a complaint filed Monday in Maricopa County Superior with such charges as taking the identity of another and forgery, both felonies.

The defendants were identified in the complaint as Emigdio Munoz Gonzalez, Fernando Abundez Gonzalez, Victor Vargas Moran and Valentin Villanueva-Fernan.

The crimes were committed between 2002 and this month, according to the complaint.

Sheriff’s detectives arrested 10 people at two Uncle Sam’s locations, one on a felony warrant. They suspected during a nine-month investigation that as many as 121 people gained employment at the Uncle Sam’s chain since last October through the use of falsified documents and ID theft, spokesman Chris Hegstrom said.

During the raids, deputies attempted to find 70 of the employees working either at the Peoria location at 83rd Avenue and Union Hills Drive or the north Phoenix location at 32nd Street and Shea Boulevard.

Insurance Fraud Lafayette, LA Man Pleads Guilty

Oliver Lockett, 44, of Lafayette, pleaded guilty Wednesday before U.S. District Judge Richard T. Haik to staging vehicle accidents in order to defraud insurance companies.

According to evidence presented at the guilty plea, Lockett admitted to causing multiple vehicle accidents from 2003 until March 10, 2010, the U.S. Attorney’s Office reports. He also admitted to conspiring with others who would serve as guest passengers who knew ahead of time there would be a crash.

Some of the co-conspirators traveled in separate cars behind Lockett and would pose as independent witnesses. As a result, the defendant made numerous claims to insurance companies for property damage and personal injury.

Lockett faces up to five years in prison, three years of supervised release, a $250,000 fine and restitution for the wire fraud count. A sentencing date was not set.
