Fraud Investigation Physician in Chicago Indicted

Federal prosecutors say a physician with offices in Michigan City and Chicago submitted more than $3 million in false medical claims to private insurers and the government.

The U.S. attorney’s office in Chicago says Paul Madison, 59, was indicted on 11 counts, including health care fraud. The charges are linked to Madison’s practice at Watertower SurgiCenter in Chicago.

Madison is also medical director of Midwest Pain Clinic in Michigan City.

Prosecutors allege that Madison directed Watertower’s staff to submit bills for unperformed procedures to private insurers and the U.S. Department of Labor’s Office of Workers’ Compensation Programs.

Each count carries a maximum penalty of 10 years in prison.

Missing Person Mom Gets Closure 21 Months Later

Lynnard Burton was on his way home to Lexington when he disappeared in the spring of 2011. A year later, a body was found in an abandoned apartment that matched his description. Just a couple weeks ago, Lynnard’s mom says the coroner confirmed that man was her son. She talked with LEX 18 about what an emotional year it’s been.

Benita Burton says her pain started 21 months ago. For almost two years she’s been searching for the answer to one question: what happened to her son, Lynnard?

“It’s been a hard year. Just accepting everything we’ve been through, him being gone,” she says.

March 2011: Lynnard had just finished serving time at a prison in Ashland, Kentucky. He was supposed to come home by bus. Despite all his letters telling Benita he was excited to return, Lynnard never showed up.

Then in May, a decomposing body was found at the abandoned Pennington Place Apartments on Codell Drive. Lynnard’s release papers were found next to it.

“Why was he there? How he got home, what kind of frame of mind was he in when he got here? Was he sick? Just a lot of stuff I don’t know why he didn’t come home and let us know he was here,” Benita says.

She may never know the answers to those questions, but she has gotten closure. Two weeks ago, she says she got a call from the coroner. Benita says the man found in those apartments has been positively identified through DNA testing as her son.

“I thought we’d be ready for it, I was ready for it, but I wasn’t. It was hard,” she says.

But she says it’s what she needed to hear to say goodbye. Benita has planned a memorial service for her son, next saturday, January 5, at 11:30am, at First African Baptist Church in Lexington.

“I think that the pain will sooner or later lighten up a little bit. But I will always have him in my heart.”

Cold Cases KY State Police Searching for Answers

Kentucky State Police are renewing their effort to solve unsolved cases by asking the public for assistance with leads.

In 2010, they distributed more than 8,000 decks of playing cards to prisons across Kentucky depicting unsolved case information.

The deck of cards highlighted 52 unsolved Kentucky homicide or missing person cases.
Since that time, three of those cases have been solved.

Kentucky State Police hopes that sharing information with the public again will provide fresh leads that can help solve these cases and bring some closure to families.

Accident Reconstruction Lexington Marine Killed in Wed. Crash

The family of a Lexington marine killed in a crash the day after Christmas says he was on his way to surprise his girlfriend and her kids with presents when he wrecked.

33-year-old Jason Grizzell died Wednesday morning on U.S. 27 in northern Garrard County.

“I don’t think there was anything in this world that brought him more joy than seeing someone else smile,” said Grizzell’s girlfriend, Beverly Vann.

Vann showed LEX 18 the necklace Grizzell bought for her. He was never able to deliver it.

As Grizzell drove through Garrard County on his way to Lexington, investigators say, for some reason, his car crossed the center line on U.S. 27, colliding head-on with another vehicle, driven by 31-year-old Islam Ali. Ali was taken to UK Hospital where Thursday afternoon he remained in ‘fair’ condition.

Grizzell, a marine and young dad, was pronounced dead on the scene.

“Besides being my boyfriend, he was my best friend. He was my soul mate. Nobody understood me like he did,” Vann said.

“He was just a wonderful young man. I’m so proud, so very proud he’s my son,” said Grizzell’s father, James. “I was so numb, I couldn’t even think… just the fact that he’s gone from this world. The fact that I won’t see him tomorrow. I’ll never see him again in this life.”

Grizzell’s family said he lived to help others. He would bring blankets to the homeless and buy groceries for those less fortunate.

“My heart aches for the wedding that we will never have and for the life we will never be able to live,” said Vann.

Grizzell’s visitation will be Friday from 11 a.m. to 2 p.m. at Houstonville Baptist Church in Lincoln County, with the funeral following that service.

Missing Person Lonnie Jackson of Lower East Side NY

Police have issued a missing senior bulletin for Lonnie Jackson, who lives near Delancey and Columbia streets on the Lower East Side. He’s 78 years old, weighs 275 pounds and has grey hair and brown eyes. Mr. Jackson was last seen yesterday near 7th Avenue and West 78th Street. He was wearing a brown hat, brown dress suit and he was carrying a wooden cane. If you can help, call 9-1-1.

Private Detective Lincoln County Teen Shooting Accident

The Lincoln County coroner confirms the shooting death of a 16-year-old was accidental.

Police say Cody Mesaris was in his room with two friends over the weekend when he accidentally shot himself in the head. His mother was home at the time, but says she did not hear the gun go off.

Private Detective 22 Horses Found in State Trooper’s Front Yard

Officials continue to search for the owner of 22 horses that mysteriously showed up in a Kentucky State Police trooper’s Bath County front yard on Christmas day.

Police say Sgt. Jim Bowling discovered the horses loose in his yard around 7:30 p.m. Tuesday. Officials say it appears the horses were loose for some time before the trooper noticed them, and they had been wandering on a nearby highway. Bowling corralled all 22 horses in a fenced lot on his farm. Police say he contacted nearby farms, but nobody knew the owner of the animals.

Accident Reconstruction Woman in Jessamine County Crash has Died

A woman injured in a Jessamine County crash Wednesday has died.

The driver of a Corvette lost control along Richmond Avenue and hit an SUV. EMS took the driver of the SUV and a passenger in the car to the hospital.

Police say the driver of the SUV, Billie Blevins, 59, of Nicholasville, later died.

Investigators still haven’t determined the cause of the accident.

Missing Person George Ortega of Albuquerque, NM

Albuquerque police are looking for an elderly man with dementia who was last seen Downtown at approximately 2:30 a.m on Christmas Eve.

They say George Ortega, 70 is approximately 5’4 and weighs 130 lbs.

He has gray hair and brown eyes. When last seen, he was wearing a dark blue coat, blue jeans, and white high top tennis shoes.
