Missing Person Frederick Lee Perry of Fredericksburg, Virginia

Fredericksburg Police is searching for a missing man last seen mid-day Thursday in Fredericksburg.

Frederick Lee Perry, 69, is 6’1”, 180 lbs., with white hair and blue eyes. Tattoos include a chicken foot on his left hand and “Linda” on his right arm, among others.

He was last seen in the 1500 block of Jefferson Davis Highway, wearing khaki pants, a light weight khaki colored jacket, and dark shoes.

 

 

Family members say he went to pick up a prescription from a nearby CVS, but he never picked up the prescription and he did not return home.

Private Detective: Man Arrested After 4 Year Old Suffers Head Injury

Lexington Police are investigating how a 4-year-old boy suffered a head injury which sent him to the emergency room.

They tell LEX 18 that the boy was home with his mother’s boyfriend, Carl Carpenter, on Crosby Drive Thursday afternoon when he was injured.

Officers say he was in and out of consciousness, so the boy was taken to a neighbor where they called 911.

There is no word on the boy’s condition.

Investigators say Carpenter was arrested on an existing warrant but he has since been charged with criminal abuse. According to his citation, Carpenter admitted to pushing the little boy down the stairs.

Private Detective: Alleged Uber Attacker Sues Driver For $5 Million

The former Kentucky man accused of attacking a California Uber driver in November is suing the driver for millions of dollars.

Days after the video went viral with more than 2 million views, Benjamin Golden, spoke about the incident.

“It’s not me in the video it’s not me and it was hard to watch.”Golden said. “I’m ashamed and what’s taken me so long to talk to anybody is because I’ve been scared.”

But just two months after that night, Golden is suing the Uber driver, Edward Caban,  for $5 million.

“I lost my job I don’t have a lot of resources behind me,” Golden said.

Golden was an executive for Taco Bell. Before that, he worked in the Public Relations department for YUM brands in Louisville and acquaintances said he often traveled to Lexington.

Golden was fired from his new job in California soon after the video went public.

That’s only part of the reason Golden is suing Caban. The other reason, Golden’s lawyer claims the video was recorded illegally and that his client didn’t know the camera was on.

In a court hearing, Golden said that he denies causing any damage to the driver. saying Caban is responsible for any injuries that night which is contrary to what Caban remembers.

“He was grabbing my head and was trying to smash it against the window,” said Caban. “He was very aggressive, he was swearing at me, he was fading in and out of consciousness.”

As for the video that made national headlines, Golden’s lawyer says she will try to have it thrown out of his criminal case saying it was recorded illegally.

According to the California penal code, a person cannot record a conversation without consent.

However, the driver’s attorney believes that only applies to audio recording, not video recording

Accident Reconstruction Woman Charged With Wanton Endangerment and DUI After Crash

A 20-year-old woman was taken to jail for wanton endangerment following a crash Thursday in Richmond.

Faith Nicole Robinson was arrested after Richmond Police responded to the report of a vehicle fire on Hampton Way.  Upon arrival, officers found that a vehicle fully engulfed in flames.

Robinson, identified as the driver, and two passengers escaped without any serious injuries.  It was determined that Robinson’s vehicle had run off the roadway, striking an electrical box, a metal light pole, and a parked vehicle. After the collision the vehicle caught fire.

Officers subsequently determined that Robinson was operating under the influence of alcohol and took her into custody.

She was lodged in the MCDC and charged with DUI and wanton endangerment.

Accident Reconstruction Victim IDed in Fatal Anderson Co. Crash

The Anderson County Coroner has confirmed that one person was killed in a crash. It happened on KY 44 shortly after 9 Friday morning.

Transportation officials say there was just one car involved but that two people were inside the car. The road opened back up around 1 P.M.

The person killed in the crash has been identified as 21-year-old Justin Thurman of Lawrenceburg. Thurman was the driver of the vehicle. A passenger inside the vehicle, Kristian Baxter, age 21,  is at the University of Kentucky Hospital recovering from his injuries.

“The area where the collision occurred is a very curvy very dangerous area also whenever you get water or any moisture on the road it increases the level danger of danger in those areas,” Trooper Kendra Wilson told LEX 18.

Police say that alcohol may have been a factor.

Wrongful Death Two Sentenced in 2014 Murder of Lexington Man

Two men accused of gunning down a Lexington man in an apartment hallway in 2014 were sentenced Friday.

Chauncey Ingram and Sayvon Thompson were accused in the murder of 19-year-old Jefferey Adams. Adams was found shot to death in an apartment hallway on McCullough Drive.

Chauncey Ingram,18, received 30 years consecutively for manslaughter and robbery.

Sayvon Thompson received 20 years for criminal facilitation to commit manslaughter and robbery.

During the hearing, Ingram’s attorney’s talked about his troubled childhood.

Adams’ mother, Jessica Sweat, said that she will never get her son back.

“My son was shot down in the head twice by you, left in an apartment hallway where you went through his pockets… Poor you,” said Sweat.

Prosecutors say Adams had no idea he would be robbed when he met Ingram and 18-year-old Thompson at a Lexington Apartment.

“Something very telling came out of the facts in this case, that after the first shot, Jeffrey lay there crying for help and you say, ‘we can’t leave him like this,'” said Judge Pamela Goodwine.

Prosecutors say Ingram then shot Adams a second time, killing him. After that Ingram and Thompson left and got haircuts.

“He has two brothers, lots of friends that miss him everyday,” said Sweat. “My heart will never be the same.”

Private Detective: Woman Goes on Naked Rampage in GA Waffle House

Hash browns weren’t the only thing that some Waffle House patrons in Georgia wanted covered.

A Kennesaw woman is facing several charges after going on a rampage at a Waffle House restaurant in the nude.

The incident happened January 8 around 10:30 p.m. at the location on George Busbee Parkway.

According to police, the woman, identified as Jennifer Nicholson, stripped off her clothes, punched a woman in the face, breaking her nose, and then threw a plate at a window. She also threw several platters at patrons of the restaurant.

Nicholson is also accused of fighting back with police officers.

Nicholson is facing several assault and public indecency charges.

She’s currently being held in the Cobb County Jail.

Private Detective: Laurel Co. Deputies Investigating Attempted Robbery of Dollar Store

Deputies in Laurel County are investigating an attempted robbery.

It occurred at the Saddlebrook Dollar Store on Thursday night. The investigating detective reports that a lone male subject entered the store around 9:40 p.m.

The suspect went straight to the cashier’s desk where some customers were checking out. He approached the clerk and assistant manager behind. The male subject, wearing a hoodie and bandanna to conceal his identity, implied that he had a weapon and demanded money.

The assistant manager immediately picked up the phone calling 911 and reached to grab the bandanna off the suspect’s face, causing the suspect to flee out the front door.

The suspect was last seen running westbound toward Ky 192.

The subject in question was approximately 5’10” to 6′ feet tall with a thin build. He was wearing a dark gray hoodie and a blue bandanna and blue jeans. No weapon was ever produced.

Private Detective: KY Anesthesiologist Guilty of Unlawful Distribution of Controlled Substances

A Kentuckiana anesthesiologist pleaded guilty today in U.S. District Court to charges of unlawful distribution or dispensing of controlled substances, health care fraud, conspiracy and money laundering announced United States Attorney John E. Kuhn, Jr.

Jaime Guerrero, age 48, formerly a medical physician with offices in Louisville, Kentucky, and Jeffersonville, Indiana, pleaded guilty to 31 counts of a 35 count Superseding Indictment before U.S. District Judge Greg N. Stivers.

According to terms of the plea agreement, Guerrero will forfeit the proceeds from these offenses, including his license to practice medicine and real property owned by Guerrero Real Estate Investments, LLC. Further, the defendant will pay $827,000 in victim restitution to nine health care benefit programs.   In return, at the time of sentencing, the United States has agreed to recommend a 100 month sentence in prison, followed by a three year period of supervised release.

According to the plea agreement, from November 1, 2009, continuing through January 1, 2013, Guerrero conspired with others to knowingly and intentionally distribute and dispense, schedule II and III controlled substances to patients, without a legitimate medical purpose and beyond the bounds of professional medical practice. Beginning November 1, 2009, and continuing through May 8, 2014, Guerrero admitted to distributing and dispensing Schedule II and III controlled substances to patients (listed in counts 2-26) without a legitimate medical purpose beyond the bounds of professional medical practice. Beginning on or about January 6, 2010, and continuing through September 16, 2011, Guerrero knowingly and intentionally distributed and dispensed, and caused to be distributed and dispensed Hydrocodone, a Schedule III controlled substance, to patient S.O., without a legitimate medical purpose and beyond the bounds of professional medical practice, which resulted in S.O.’s death on or about September 24, 2011.

Guerrero further pleaded guilty to three counts of health care fraud for fraudulently billing various health care benefit programs and for submitting fraudulent claims for patient health care counseling.  Specifically, on May 26, 2011, June 15, 2011, and June 22, 2011, Guerrero saw more than 100 patients on each of the dates, by himself, and spent approximately 3 minutes or less with each patient, and fraudulently billed various health care benefit programs, for office visits at a higher code than the service provided. Between July 9, 2010 and July 22, 2010, Guerrero travelled outside of the United States and directed staff personnel to provide group counseling sessions for patients in his absence. The group sessions were then billed as individual counseling sessions, and as if Guerrero personally provided the service. Additionally, between January 1, 2008, and continuing through June 15, 2012, Guerrero falsely and fraudulently submitted over 100 claims to various health care benefit programs for office visits at a higher code than the service provided; for office visits that were not medically necessary or within the course of usual medical practice; submitting claims for services that were not sufficiently documented in the patient’s medical record; and making claims for office visits as though a physician saw the patient, when in fact, a nurse practitioner saw the patient. Guerrero agreed to pay $827,000 in victim restitution to nine health care benefit programs

Finally, Guerrero pleaded guilty to a single money laundering charge for redeeming (paying the taxes) on a building located at 1201 West Wall Street in Jeffersonville, Indiana, with $89,556.25 in cash derived from unlawful activity – the unlawful dispensing and distribution of controlled substances and health care fraud.

This case is being prosecuted by Assistant United States Attorneys Joseph Ansari and Lettricea Jefferson-Webb and is being investigated by the Federal Bureau of Investigation (FBI), the U.S. Department of Health and Human Services-Office of Inspector General (HHS-OIG), the U.S. Drug Enforcement Administration (DEA), Indiana and Kentucky Medicaid Fraud Control Units, and Louisville Metro Police Department (LMPD).

Pedophile Tracking Amarillo, TX Man Indicted

An Amarillo man was indicted last month in Potter County on two felony counts of continuous sexual abuse of a child under the age of 14 and one count of indecency with a child, according to court documents released this week.

Court documents claim Jesus Jose Herrera, 34, abused children from 2008 to April.

Officers were alerted to claims of continual sexual abuse after interviews at the Bridge Children’s Advocacy Center, which assists abused children and non-offending family members, according to court documents.

 

Herrera was in Potter County jail Tuesday in lieu of $175,000 bond.

If convicted, Herrera faces up to life in prison and a $30,000 fine for the first- and second-degree felony charges.
