Fraud Investigation Hospice Owner Pleads Guilty

The owner of a Mississippi hospice has pleaded guilty in a multimillion dollar Medicare fraud investigation.

Regina Swims-King was charged in June in a 37-count criminal indictment related to the actions of Angelic Hospice in Greenwood.

She pleaded guilty Thursday to one count in U.S. District Court in Oxford.

Prosecutors said Angelic Hospice billed Medicare for millions of dollars services that were never rendered, sometimes involving forged documents.

The hospice billed Medicare more than $11 million from 2007 to 2012, according to court records.

Authorities said Swims-King used the money to fund a lavish lifestyle, including at least 20 cars.

Prosecutors said a hospice recruiter went door-to-door asking whether residents needed their blood pressure checked. The hospice then used their information to bill Medicare for services, prosecutors said.

In one example, authorities said the hospice billed thousands of dollars on behalf of someone who did not need hospice care and was healthy enough to walk several miles a day.

Some of the claims submitted to Medicare had the forged signatures of alleged beneficiaries and doctors, according to the indictment.

Missing Person Reward Offered for Missing Carbondale, IL Man

The City of Carbondale Police Department continues to investigate missing person Pravin Varughese, who was last seen in the 600 block of West College Street at 11:00 p.m. on 02-12-14.  The family is offering a reward of at least $5,000 for information leading to the recovery of Varughese.  Anyone with information is encouraged to call the Carbondale Police Department or Southern Illinois University Department of Public Safety.

Missing Person 7 Month Old Girl in Helena, Montana

The Montana Department of Justice has issued a Missing/Endangered Person Alert for Willow Dawn O’Neill, a 7-month old girl from Helena.

The Helena Police Department reports that she was taken during a disturbance by her father, Joshua O’Neill, who reportedly does not have the proper means to care for the child.

Joshua is a 42-year old man, 5 ft 10, 185 pounds, with brown hair and hazel eyes.

They are possibly in a dark green Pontiac or red Nissan pickup with a white tailgate and may be in the Canyon Ferry area.

The Helena Police Department is currently applying for a warrant for Joshua O’Neill for criminal endangerment.

Wrongful Death Middlesboro Man Accused of Killing Son

A Middlesboro man is dead, and his father is charged with killing him.

Police say Gary Baker, Jr. stabbed his son Joshua Baker to death.

“It’s not every morning you wake up and hear you’re not going to see your best friend again,” said Tyler Cox, Josh Baker’s friend.

Cox’s friend, 24-year-old Josh Baker, died Thursday night. Police say Baker’s own father stabbed him to death during an argument.

Cox does not know why.

“Josh was a good hard-working fellow. He was usually happy go lucky for the most part,” said Cox.

Middlesboro Police say they responded to a call at a home on Evans Drive at 10:40 Thursday night. Once there, officers say they found Josh Baker with a stab wound in his chest. He died at the scene.

Police charged his father, 51-year-old Gary Baker, Jr. with murder. Baker, Jr. pleaded not guilty to the murder charge. His defense attorney had no comment. He has a preliminary hearing scheduled for February 25th. Jail officials say bond is $500,000.

Cox is still trying to come to grips his best friend is gone.

Accident Reconstruction Teen Dead After Lee County Crash

One person is dead after an accident in Lee County Friday afternoon.

Around 4:05 p.m. Beattyville police responded to a motor vehicle versus motorcycle collision at the intersection of KY 11 and Shoemaker Ridge Road, just north of Beattyville.

Jordan Flinchum, 15, was driving a motorcycle onto KY 11 from Shoemaker Ridge Rd and pulled into the northbound path of another motorist, Luke Phillips, 27, who was driving a motor vehicle.

Flinchum was flown to Hazard Regional Medical Center where he later passed away as a result of his injuries. Beattyville police report he was not wearing a helmet at the time of the incident.

The collision is still under police investigation at this time.

Missing Person Silver Alert in West Virginia

The West Virginia State Police has issued a Silver Alert and The Huntington Police Department has opened a missing person investigation concerning Joan Gardner (84) who lives in the south Queens Ct. area of Huntington. Her family has been trying to reach her since Friday, Feb. 14,  around 3:30 pm, when they went to her home. She was not home and her vehicle was  missing.

Wrongful Death Missing Trucker Found Dead in Kansas City Freezer

Police in Kansas have identified a man who was found dead in a freezer.

On Thursday, authorities announced that Lawrence P. Muirhead, a trucker who went missing Sept. 28, was the man found stuffed in a freezer inside a residential garage earlier this week. His death is being investigated as a homicide.

Kansas City Police are still trying to piece together what happened to the 53-year-old Muirhead, who was discovered on Sunday by a woman at the home that she is renting.

The woman, whose name is withheld, told police that her boyfriend warned her in October not to look in the locked freezer, according to KCTV5. Muirhead was considered missing after his tractor-trailer was found abandoned in the parking lot of a Merriam, Kan., K-Mart.

The boyfriend has been in the hospital since December, battling terminal cancer.

But on Sunday, curiosity got the best of the woman, who smashed the lock with a shovel and made the grisly discovery. Police said Muirhead had been shot in the head before being placed in the freezer.

The owners of the property said they had no idea about the freezer, which appears to have been placed in the garage by the renter’s boyfriend.

Muirhead had been making a trip from Pennsylvania to Nogales, Ariz., when he disappeared. His family filed a missing person report after he didn’t show up at his destination Oct. 1.

The trucker’s 89-year-old mother told KCTV5 that she’d last spoken with her son on Sept. 27, and that his wife had talked with him on the phone the following day.

“He said on the phone, ‘I’m around Kansas City in a heavy traffic situation, so I will call you later.’ And that was the time she ever talked to him,” Muirhead’s mother, Alyke Muirhead told the station. “We know nobody who was threatening him. But obviously there was something, and we don’t know, but we want to know. Somebody did him in, and we don’t know why.”

She said her son, a former body builder, had hoped to own his own trucking company.

“Now we at least know where he is. He’s not just out there being held by someone or something awful,” Alyke Muirhead told KSHB.

Missing Person Amanda Reynolds of Mascoutah, IL

The Illinois State Police issued an endangered person advisory for a missing teen early Friday afternoon.

Amanda Reynolds, 14, was last seen Thursday, according to police.

Authorities believe she and another person, Trevor Villapando, 17, could be possibly on their way to California or Mexico.

She is described as being 5 feet, 1 inch and weighing 105 pounds. She wore a brown, green and gray North Face jacket and blue jeans. Villapando is described at 5 feet, 8 inches, 180 pounds, with brown hair and green eyes. They are traveling in a black 2003 Honda Accord with a license plate of N985555.

Private Detective Madison County Man Charged with Drug Trafficking

A Madison County man was arrested Thursday after a joint investigation between Madison County Sheriffs and Berea Police Department.

The departments executed a search warrant at the home of George Richardson on Hager Drive in Richmond.

Officers found oxycodone, methadone and suspected crystal methamphetamines. They also found $6,000 in cash and a handgun.

Richardson has been charged with three counts of trafficking a controlled substance first degree.

He is being lodged at the Madison County Detention Center.
