COLUMBUS, Ohio — An alert has been issued for a West Jefferson man after he disappeared from his nursing home Monday morning.
Authorities are asking for residents of Central Ohio to be on the lookout for 46-year-old Terry Underwood.
Underwood was last seen at The Arbors West Nursing home at 375 West Main St. in West Jefferson at about 10:30 a.m. Monday.

Underwood is described as a white male who is 5 feet 6 inches tall and weighs 177 pounds. He has brown hair and blue eyes and was last seen wearing blue jeans, a navy blue T-shirt and brand new white and blue Nike tennis shoes.
Officials said Underwood has a mental disability, is diabetic and in need of his medication. His bilateral lower extremities have been amputated and he walks with a walker. The walker is maroon in color with a black seat.
