Insurance Fraud Camden County, NJ Father Pleaded Guilty

A Camden County father of twin boys admitted he submitted more than $140,000 in fraudulent claims for hospital stays his sons never had.

Steven Herder, 41, of Lindenwold, pleaded guilty to second-degree insurance fraud for filing five false claims to his insurer, American Family Life Assurance Company (“Aflac”) between June 2012 and November 2014. The boys were aged 6 months to 3 years old when the claims were submitted.

Under the terms of the plea agreement, the state will recommend a three-year prison term when Herder is sentenced on November 4th.  Herder must also pay $140,000 in restitution to Aflac. The plea hearing was held before Camden Superior Court Judge John T Kelly.

“What makes this case especially distasteful is that this man not only cheated and lied to his insurance carrier, he used his young sons as props to carry out the illegal scheme,” said Attorney General Christopher S. Porrino. “We will seek a prison sentence for Mr. Herder in response to his blatant fraud.”

The claims sought payment for five stays – ranging from 11 weeks to 26 weeks – at West Jersey Hospital in Voorhees for one or the other of his twin boys.  Herder forged three doctors’ signatures on the fake hospital records, which he had manufactured on a computer, according to prosecutors.

Aflac paid approximately $140,000 in total for four of the claims and denied the fifth, valued at $30, 250.

“Health care fraud is a serious crime that drives up insurance costs for honest people with legitimate illnesses,” said Acting Insurance Fraud Prosecutor Christopher Iu. “ Anyone caught cheating New Jersey’s health care system will face serious consequences, as this defendant has learned.”

Deputy Attorney General Colin Keiffer presented the case to the grand jury. Detectives Kristi Procaccino, Megan Flanagan, and Kahlil McGrady coordinated the investigation. Assistant Bureau Chief Christina Runkle and Analysts Theresa Worthington and Terri Drum assisted the investigation. Acting Insurance Fraud Prosecutor Iu thanked Aflac’s Special Investigations Unit for the assistance in the investigation.

Acting Insurance Fraud Prosecutor Iu noted that some important cases have started with anonymous tips. Missing Person Makayla Lanning of Haywood County, NC

Detectives with the Haywood County Sheriff’s Office are seeking the public’s assistance in locating a missing juvenile who was last seen in Clyde at approximately 8:20 p.m. Oct. 5, 2016.

Makayla Lanning, 17, is 5’4” tall and approximately 130 pounds. Lanning has brown eyes and brown, straight hair that falls past shoulder length.

The last person who had contact with Lanning said she was wearing dark spandex pants, a dark shirt and a grey zip up hoodie.

Wrongful Death KSP Investigating Fatal Shooting in Carroll Co.

KSP Troopers are investigating a fatal shooting of a man and woman at the Double H Farms in Carroll County.

On Friday around 12:51 p.m., Carroll County dispatch received a 911 call in reference to a shooting on US 42 East.

Andy McKinney, 47, and Kerry McKinney, 45, both of Owenton, were found at the scene. Both had suffered fatal gunshot wounds and were pronounced dead at the scene, according to KSP.

Autopsies are scheduled for Saturday at the Medical Examiner’s Office in Louisville.

Missing Person $20k Reward Offered For Tacoma Girl Missing Since 1979

Angela Mae Meeker vanished in 1979 just two days before her 14th birthday.

Now the FBI is offering a reward of up to $20,000 to resolve the missing-person case.

The Sound Sound Cold Case Working Group, which uses FBI resources and investigations by such agencies as the Tacoma Police Department, is trying to resolve the case,

“The FBI hopes that this reward is an additional incentive for people to come forward with information,” said Senior Supervisory Resident Agent Kelly Smith, who oversees the FBI’s criminal investigations in Pierce County, said in news release. “Even the smallest piece of information may contribute to solving that puzzle. The South Sound Cold Case Working Group is committed to piecing together a picture of what happened 37 years ago.”

Angela Mae Meeker would be 51 today.

She was last seen on July 7, 1979. She left her South Tacoma home to go to the Tacoma Mall to buy a friend a birthday card. She was reportedly seen at a party that night. She didn’t return home.

She had hazel eyes, brown-blond hair. She weighed 100 to 110 pounds and was 5 feet 1 in inch tall.

At the time of her disappearance, Angela Mae Meeker was last seen wearing a black floral print shirt, blue jeans, a satin jacket, sandals and a black felt hat with red feathers on it.

Private Detective: KSP Looking For Suspect in Sexual Assault Investigation in Bedford

KSP is seeking assistance from the public in identifying a suspect in a sexual assault investigation.

Troopers said that they received a call about a woman who had been sexually and physically assaulted in the community of Bedford, Kentucky on September 20 around 10:00 p.m.

The incident occurred on West Avenue and troopers said the victim walked to the Valero gas station on US 42 to report the incident.

Troopers said that during the course of the investigation, the victim was able to provide investigators with a description of her assailant.

He is described by troopers as a white man of medium build with dark hair cut short on the sides and long on top.

Wrongful Death Woman Accused of Masterminding Murder of UK Grad Extradited Back to the US

The woman investigators say was behind the murder-for-hire plot of a University of Kentucky graduate has been extradited back to the United States from Mexico.

Brenda Delgado faces capital murder charges for allegedly orchestrating the murder of Kendra Hatcher in an Uptown Dallas parking garage over a year ago.

Hatcher was in a relationship with Delgado’s ex.  Investigators have said Delgado was jealous of the beloved dentist and arranged for two people to kill Hatcher in return for money and drugs.

Both alleged co-conspirators have been arrested, the FBI said.

Delgado holds Mexican citizenship and fled the US after being interviewed by investigators.  A federal arrest warrant was issued last October.

Pedophile Tracking Oregon City, OR Man Arrested

An Oregon City man has been arrested for alleged sexual abuse of a minor in Corvallis.

The Benton County Sheriff’s officer learned about a female minor who was reportedly involved in a sexual relationship with 61-year-old Donald Grey in September.

Grey was arrested on October 6 in Oregon City for charges of encouraging child sex abuse I, encouraging child sex abuse II, 12 counts of sexual abuse III and 12 counts of contributing to the sexual delinquency of a minor.

Detectives are still investigating and evidence indicates there may be more victims in other areas. Law enforcement asks anyone with information about possible victims to contact the Clackamas County Sheriff’s office tip line at 503.723.4949.

Private Detective: Police Investigating After Shot Fired Into Lexington Home

Lexington Police are investigating after a shot was fired into an apartment overnight.

Police say a man was home when a stray bullet came through the window of his Raleigh Road apartment. He was not hit.

Private Detective: Laurel Co. Man Accused of Shooting at Officer

A Laurel County man is charged with four counts of wanton endangerment after he allegedly fired a gun at two state troopers.

Sheriff’s officials say that Roy Hibbard, 63, was arrested after he stepped out on his porch and fired a shot at the officers as they were conducting an investigation near his house. The officers had been searching the area for a suspect who had fled from officers earlier in the week, but no one has indicated whether or not that suspect was Hibbard.

Officers took him into custody without incident and recovered a Glock 9mm pistol from him. Hibbard told officers he had taken hydrocodone earlier in the evening.

Hibbard was lodged in the Laurel County Detention Center.

Missing Person Joel Whitener Hart of Caldwell Co., NC

The Caldwell Co. Sheriff’s Office is asking for the public’s help in locating 51-year-old Joel Whitener Hart. Mr. Hart was last seen at about 9:30 a.m. on Friday, September 30, at his home on Deal Lane in Hudson. He had planned to visit a friend in the Rhodhiss area before a scheduled 11 a.m. appointment in Morganton.

Mr. Hart is 5’8” tall and weighs 220 pounds. He was last seen on a silver and blue moped.
